断脐时间对地中海贫血及葡萄糖-6-磷酸脱氢酶缺陷症产妇母儿分娩结局的影响

摘　　要：目的探讨断脐时间对地中海贫血、葡萄糖-6-磷酸脱氢酶缺陷症产妇母儿分娩结局的影响。方法将携带地中海贫血基因产妇100例、葡萄糖-6-磷酸脱氢酶缺陷症产妇80例、正常产妇100例均分为早断脐组和晚断脐组,早断脐于胎儿娩出后少于10 s断脐,晚断脐组等待脐带搏动停止后断脐,比较两组产后出血量、新生儿脐血血红蛋白值、出生后3 d最高胆红素值以及新生儿贫血和蓝光治疗例数与蓝光治疗时间。结果 3组新生儿晚断脐组产后出血量、胎盘剥离时间、新生儿贫血发生率均低于早断脐组(P<0.01);地中海贫血及正常产妇晚断脐组新生儿胆红素水平较低;正常产妇组中晚断脐组新生儿需蓝光治疗时间减少(P<0.05)。结论对地中海贫血和葡萄糖-6-磷酸脱氢酶缺陷症的产妇分娩时采用晚断脐方法是安全的,能减少产后出血量且不增加新生儿胆红素水平,值得临床推广应用。Objective To determine the influence of umbilical cord cutting time on delivery outcome of women with thalassemia and Glucose - 6 - phosphate dehydrogenase deficiency. Methods Totally, 100 delivery women carried gene of thalassemia,80 delivery women with Glucose - 6 - phosphate dehydrogenase deficiency and 100 normal delivery women were equally divided into the delayed cord clamping group and the early cord clamping group. Ompha- lotomy of neonates was conducted less than 10 seconds after fatal delivery in the early cord clamping group, while omphalotomy of neonates was conducted when the umbilical pulse stopped in the delayed cord clamping group. Postpartum hemorrhage, hemoglobin of neonatal umbilical blood, the highest value of bilirubin within 3 days after birth, neonatal anemia, cases and duration of blue phototherapy were compared between the two groups. Results Postpartum hemorrhage, the time of placental separation and the incidence of neonatal anemia in the delayed cord clamping group were lower than the early cord clamping group（ P 〈 0.01 ）. Neonatal bilirubin in the delayed cord clamping group of delivery women with thalassemia and normal delivery women was lower. Duration of blue phototherapy in the delayed cord clamping group of normal delivery women decreased （ P 〈 0.05 ）. Conclusion It＇s safe to delay umbical cord cutting time for women with thalassemia and glucose- 6- phosphate dehydrogenase, can reduce postpartum hemorrhage, and the level of bilirulin remain stable.
