摘 要:目的总结颅底及骶尾部脊索瘤的CT与MRI影像学特征。方法回顾性分析31例脊索瘤患者的CT与MRI影像学资料。结果脊索瘤位于颅底19例,骶尾部12例,其中原位复发21例;CT与MRI显示病灶呈椭圆形11例,不规则分叶状16例,结节状4例;CT示病灶呈不均匀低密度影,27例病灶内见点、片状高密度影,21例可见囊变、坏死,15例病灶向周围解剖结构钻缝样生长;MRI示T2WI呈明显高信号及"蜂房样"表现;CT及MRI增强扫描后病灶呈不均匀轻、中度强化。结论脊索瘤CT、MRI表现有一定特征,对术前诊断有较大价值。Objective To discuss CT and MRI imaging characteristics of skull base and sacrococcygeal chordoma.Methods The CT and MRI imaging data were retrospectively analyzed in 31 patients with chordoma.Results Chordoma was located in skull base in 19 patients and in sacrum in 12,in which 21 patients were found recurrence in situ.CT and MRI showed oval-shape lesions in 11,irregular lobulated-shape lesions in 16 and nodular lesions in 4.CT showed uneven low-density shadow in all lesions,in which lesions were found spot-or lamellar-shaped high-density shadow in 27 patients,visible cystic change and necrosis were found in 21,and lesions showed growth in seam drill toward the surrounding anatomic structure in 15.MRI showed a high signal intensity on T2 WI and honeycomb-like image.After the CT and MRI enhancement scanning,lesions showed mild-to-moderate uneven reinforcement.Conclusion Chordoma has some special characteristics on CT and MRI imaging,which is of great value to the preoperative diagnosis of chordoma.
