摘 要:目的 探讨丹参酮ⅡA对心肺复苏大鼠脑水通道蛋白4(AQP4)表达及脑水肿的影响.方法 将78只SD雄性大鼠随机分为3组:假手术组(6只),对照组(36只),丹参酮ⅡA干预组(36只),采用窒息致大鼠心脏骤停动物模型,丹参酮ⅡA干预组复苏即刻予以丹参酮ⅡA 15 mg/kg静脉推注,对照组予以等量0.9%氯化钠注射液静脉推注,在自主循环恢复后(ROSC)1、6、12、24、48、72 h断头取脑,应用免疫组织化学染色法检测脑组织AQP4表达,干湿重法测量脑组织含水量.结果 对照组在ROSC后1、6、12、24、48、72 h各时点脑AQP4蛋白的表达分别为10.2±1.9、13.4 ±2.1、15.1±2.0、20.5±3.0、17.6±2.9、14.5±2.2,均高于假手术组的7.3±1.2,差异均有统计学意义(均P<0.01);丹参酮ⅡA干预组在ROSC后1、6、12、24、48、72 h各时点脑AQP4的表达分别为9.8±1.2、11.0±1.9、14.3±2.2、17.1±2.5、16.1±1.7、13.6±2.2,均低于对照组同时点,差异均有统计学意义(P<0.01或P<0.05).对照组在ROSC后1、6、12、24、48、72 h各时点脑组织含水量分别为(78.22±0.23)%、(78.39±0.16)%、(79.11±0.24)%、(79.43 ±0.20)%、(78.67±0.21)%、(78.06±0.20)%,均高于假手术组的(77.28±0.09)%,差异均有统计学意义(均P<0.01);丹参酮ⅡA干预组在ROSC后1、6、12、24、48、72 h各时点脑组织含水量分别为(77.60±0.16)%、(77.96±0.21)%、(78.17±0.21)%、(78.77±0.28)%、(78.05±0.23)%、(77.61±0.23)%,均低于对照组同时点,差异均有统计学意义(P<0.01或P<0.05).结论 丹参酮ⅡA能降低大鼠心肺复苏后脑组织AQP4的表达,减轻脑水肿.Objective To investigate the influence of tanshinone Ⅱ A on the expression of aquaporin-4 (AQP-4) and cerebral edema after cardiopulmonary resuscitationin rats.Methods Totally 78 Sprague Dawley male rats were randomly divided into sham group(n =6),control group (n =36) and tanshinone Ⅱ A intervention group (n =36).Cardiac arrest was induced by choking models in control group (n =36) and tanshinone Ⅱ A intervention group;tanshinone Ⅱ A intervention group received intravenous injection of tanshinone Ⅱ A (15 mg/kg) and control group was injected with the same amount of 0.9% sodium chloride injection at recovery instantly.The expression of AQP4 in brain tissue was measured by Immune-histochemical staining at 1,6,12,24,48 and 72 h after restoration of spontaneous circulation (ROSC);the water content of the brain was measured by wet/dry weight method.Results The expression of AQP-4 in control group at 1,6,12,24,48,72 h after ROSC was 10.2 ± 1.9,13.4 ±2.1,15.1 ±2.0,20.5 ±3.0,17.6 ±2.9,14.5 ±2.2,respectively,all significantly higher than that in sham group (7.3 ± 1.2) (P 〈 0.01);the expression of AQP-4 in tanshinone Ⅱ A intervention group at all time points was 9.8 ± 1.2,11.0 ± 1.9,14.3 ± 2.2,17.1 ± 2.5,16.1 ± 1.7,13.6 ± 2.2 respectively,all significantly lower than that in control group (P 〈 0.01 or P 〈 0.05).The water content of brain tissue in control group at 1,6,12,24,48,72 h after ROSC was (78.22 ±0.23)%,(78.39 ± 0.16)%,(79.11 ±0.24)%,(79.43 ±0.20)%,(78.67 ±0.21)%,(78.06 ±0.20)%,respectively,all significantly higher than that in sham group [(77.28 ± 0.09)%] (P 〈 0.01);the water content of brain tissue intanshinone Ⅱ A intervention group was (77.60 ±0.16)%,(77.96 ±0.21)%,(78.17 ±0.21)%,(78.77 ± 0.28) %,(78.05 ± 0.23) %,(77.61 ± 0.23) %,respectively,all significantly lower than that in control group (P 〈 0.01 or P 〈 0.05).Conclusion Tanshinone Ⅱ A exert can reduce t
