摘 要:目的探讨血清标记物STC-1在乳腺癌患者癌组织和外周血中的含量变化及其临床意义。方法采用荧光定量逆转录聚合酶链反应(RT-PCR)检测80例乳腺癌患者外周血、40例乳腺良性肿瘤患者外周血及30例正常人外周血中STC-1的表达水平,并对其中60例乳腺癌患者的糖链抗原125(CA-125)等血清肿瘤标志物进行比较分析。分析比较检测过程中STC-1与CA-125的特异性与敏感度。结果 80例乳腺癌患者外周血中STC-1的阳性表达率为43.8%(35/80);而40例乳腺良性肿瘤患者外周血中STC-1的阳性表达率仅为5.0%(2/40);30例健康人中无1例阳性表达。乳腺癌患者与乳腺良性肿瘤患者、乳腺癌患者与健康人的外周血STC-1基因表达水平差异有统计学意义,乳腺癌患者组显著高于乳腺良性肿瘤组及健康组。在乳腺癌的检测过程中:STC-1的特异度明显低于CA-125,而其敏感度却明显高于CA-125;二者联合使用时检测的特异度与敏感度均显著提高(P<0.05)。结论 STC-1可能在乳腺癌发生发展中起重要作用,乳腺癌外周血STC-1的含量升高可以作为一个癌变的高危指标。Objective To investigate the expression level of stanniocalcin-1 ( STC-1 ) in peripheral blood of patients with breast cancer and discuss its clinical value as circulating tumor marker. Methods The fluorescent quantitation reverse transcription polymerase chain reaction (RT-PCR) was used to detect STC-1 in the periph- eral blood of 80 patients with breast cancer,40 patients with breast benign tumors and 30 healthy subjects, and serum carcino-embryonie carbohydrate antigen 125 ( CA-125 ) were detected among 60 patients with breast cancer and analyzed comparatively. Results The positive rate of STC-1 in 80 patients with breast carcinoma was 43.8% (35/80) ,the positive rate of STC-1 in 40 patients with benign tumors was 5.0% (2/40) ,but there was no expression of STC-1 in 30 healthy patients. The expression level of STC-I in peripheral blood of patients with breast cancer was higher than that of breast benign tumors and that of the healthy subjects with statistical significance. In the detecting process of the breast cancer, the STC-1 was of lower specificity than CA-125;while its sensitivity is higher than that of the CA-125. However, both the specificity and sensitivity were remarkably increased when the STC-1 and CA-125 were detected in combination (P 〈 0.05 ). Conclusions STC-1 protein expression played an important role in breast cancer initiation and progression, and may be a potential biomarker for prognosis assessment of breast cancer
