胃腺癌组织AnnexinⅡ和VEGF蛋白表达的临床意义

摘　　要：目的探讨胃腺癌组织中AnnexinⅡ、血管内皮生长因子(vascular endothelial growth factor,VEGF)蛋白表达及其在胃癌发生、发展及浸润、转移中的作用。方法免疫组化SP法检测2013-01-01-2013-09-30山东省肿瘤医院病理科50例胃腺癌组织及其癌旁正常组织(距癌组织≥3cm)和50例萎缩性胃炎组织中AnnexinⅡ、VEGF蛋白的表达,分析2种蛋白的表达与胃腺癌临床病理学特征的关系。结果胃癌组织中AnnexinⅡ蛋白的表达阳性率为78.00%(39/50),明显高于萎缩性胃炎的36.00%(18/50),差异有统计学意义,χ2=16.320,P<0.001;也明显高于正常组织的14.00%(7/50),差异有统计学意义,χ2=38.688,P<0.001;萎缩性胃炎组织AnnexinⅡ蛋白的表达阳性率和正常组织比较,差异也有统计学意义,χ2=5.333,P=0.021。胃癌组织中VEGF的表达阳性率为84.00%(42/50),明显高于萎缩性胃炎组织的42.00%(21/50),差异有统计学意义,χ2=17.160,P<0.001;也明显高于正常组织的20.00%(10/50),差异有统计学意义,χ2=38.502,P<0.001;萎缩性胃炎组织VEGF蛋白的表达阳性率和正常组织比较,差异也有统计学意义,χ2=4.675,P=0.031。AnnexinⅡ、VEGF蛋白在低分化腺癌的表达显著高于高-中分化腺癌(P<0.05),在伴有淋巴结转移组中的表达显著高于无淋巴结转移组(P<0.05),而与浸润深度无关;AnnexinⅡ和VEGF蛋白在胃腺癌组织中的表达呈正相关,r=0.403,P<0.05。结论 AnnexinⅡ和VEGF蛋白高表达可能在胃腺癌的发生、发展和转移过程中起重要作用。OBJECTIVE To detect the expressions of AnnexinⅡ and VEGF proteins in gastric adenocarcinoma tissues, and to investigate the role of these two proteins in the occurrence, differentiation, infiltration and transfer of gastric adenocarcinoma. METHODS Immunohistochemistry SP was used to detect the expressions of Annexin Ⅱ and VEGF proteins in the 50 cases of gastric adenocarcinoma tissues and normal tissues adjacent to cancer as well as 50 cases of atrophic gastritis tissues in Shandong Cancer Hospital from January 1 to September 30,2013 respectively, and their correlations to clinicopathologic features of gastric adenocarcinoma were analyzed. RESULTS In normal tissue, atrophic gastritis and gastric adenocarcinoma tissues, the positive expression rates were 14.00 % （7/50）, 36.00 % （ 18/50）, 78. 00 % （39/50） for Annexin Ⅱ protein, and 20.00% （10/50） ,42.00% （21/50） ,84.00% （42/50） for VEGF protein. The data showed a gradually increasing trend （P〈0. 05） ;The positive expression rates of Annexin Ⅱ and VEGF in poorly differentiate carcinoma were significantly higher than that in well and moderately differentiated carcinoma （P〈0.05）. The positive expression rates of AnnexinⅡ and VEGF protein in gastric adenocarcinoma with lymph node metastasis increased significantly compared to the group without lymph node metastasis （P〈0.05）. The high expressions of these two protein were associated with the degree of cell differentiation and metastasis in lymph node, but it was not associated with the depth of invasion; There was a positive correlation between the expression of Annexin Ⅱ protein and VEGF protein in the gastric adenocarcinoma tissues（r=0. 403, P〈0.05）. CONCLUSION The high expressions of Annexin Ⅱ and VEGF protein may play an important role in the occurrence, differentiation, infiltration and transfer of gastric adenocarcinoma.
