摘 要:目的 分析与评估实时视频录像和定期自我指导方法(以下简称“视频法”)在对医师高级创伤生命支持(advanced trauma life support,ATLS)培训中的作用.方法 选取2012年参加以色列RAMBAM创伤中心ATLS国际培训班的13名中方人员为实验组,浙江省内一家综合性三级甲等医院急诊医学科传统ATLS培训的12人为对照组,比较视频法和传统培训方法对学员ATLS知识与技能掌握情况的影响.同时,就视频法对实验组学员进行问卷调查.采用SPSS 18.0统计软件分析处理所有数据,计数资料以百分比表示,t检验比较两组之间各项目改善情况的差异,以P<0.05表示差异有统计学意义.结果 培训后两组学员在高级创伤生命支持理论知识、创伤急救技能、检伤分类、二次评估和气道管理掌握方面均较培训前明显改善,但是,实验组学员在创伤急救技能、二次评估和气道管理方面改善程度优于对照组(P<0.05),而在理论知识和检伤分类方面的改善情况两者无区别(P>0.05).实验组学员认为,视频法能够使学员较快地明确自己在ATLS救治环节中的错误点,加快自我纠正速度,减少差错率,加强成员团队协作意识和沟通能力,学员对该培训方法的接受度较高.结论 视频法与传统培训方法相比能够明显改善ATLS课程培训效果,提高学员对创伤急救技能、二次评估和气道管理等关键救治步骤的掌握,加强学员团队协作意识和沟通能力.Objective To evaluate the effect of the real-time video recording and regular selfinstruction on the education of ATLS and explore a new effective ATLS training method.Methods Thirteen trainees in the 2012 ATLS international training course following the real-time video recording and regular self-instruction training way sponsored by the RAMBAM Trauma Center in Israel and 12 trainees in the traditional ATLS training course using the traditional training way conducted by a first class trauma center in Zhejiang Province of China were respectively selected to be the experimental group and the control group.The effect was used to compare of the two different training methods on the education of ATLS to the trauma team in the emergency department.After the training,a questionnaire survey was conducted in the participants in the experimental group.Using the statistical package for social sciences (SPSS Inc,Chicago,IL,USA) version 18,Percentage was utilized to express the enumeration data and t-test was used to measure the difference between the two groups.A P 〈 0.05 was considered statistically significant.Results The test score of the trainees after the training was significant higher than that of pre-training in the theoretical knowledge of ATLS,emergency skills on trauma,triage,secondary evaluation and airway management aspects in both two groups.The difference value of the test score between pre-and posttraining on the emergency skills on trauma,secondary evaluation and airway management in the experimental group was significant higher than that of the control group (P 〈 0.05),but not on the theoretical knowledge and triage aspects (P〉0.05).The trainees in the experimental group regarded the real-time video recording and regular self-instruction as an effective training way in the improvement on the ATLS knowledge and skills,which also could enhance the cooperative awareness and communication skills of trauma team members.One hundred percent of participants accepted and was willing to choosing the
