摘 要:目的:综合分析锥形束CT(CBCT)在门诊口腔临床诊断和治疗中的应用。方法:收集2012年3月-2014年8月,在本院口腔门诊应用锥形束CT检查的患者6810例,年龄为4-89岁,对其辅助诊断、指导治疗的意义进行分析。结果:复杂下颌第三磨牙阻生经CBCT定位(3465例,50.88%);缺失牙患者经CBCT设计行缺牙种植修复(1856例,27.25%);牙列畸形通过CBCT行埋伏牙导萌术定位(538例,7.90%);牙体牙髓病经CBCT检查明确复杂根管的走形、牙根折裂早期诊断(501例,7.36%);慢性牙周炎经CBCT检查评估病变牙槽骨的吸收范围(265例,3.89%);颞下颌关节病(TMD)(112例,1.65%);外伤患者经CBCT检查发现颌骨骨折(40例,0.59%);颌下腺导管肿胀经CBCT检查发现并明确定位阳性结石(33例,0.48%)。结论:CBCT技术广泛应用于口腔门诊的临床诊疗过程中,有助于进行辅助诊断和制定诊疗方案、规避手术风险,在提高门诊临床诊疗的安全性、精确性等方面具有重要的参考价值,值得进一步推广应用。Objective:To comprehensively analyze the applications of cone-beam CT(CBCT) in diagnosis and therapy in the outpatient clinic. Methods: A total of 6810 patients receiving CBCT examination, aged 4 to 89 years old, were selected from the department of stomatology during March 2012 to August 2014. Results: 3465 patients(50.88%) with mandibular third molars were evaluated by CBCT.1856 patients(27.25%) with missing teeth received dental implant restorations by CBCT design. 538 patients from pediatric dentistry and orthodontics(7.90%) were assessed exactly by CBCT examination to help guide and expose the impacted teeth. The complex root canal morphology was clearly demonstrated by CBCT analysis in 501 cases(7.36%), showing its helpfulness for early diagnosis. 265 patients(3.89%) with chronic periodontitis identified by the absorption of alveolar bone were confirmed by CBCT. 112 cases of temporomandibular joint disorders(TMD)(1.65%) and 40 cases of the jaw bone fracture(0.59%) due to trauma were found through the CBCT examination. 33 patients(0.48%) with swollen submandibular duct were clearly diagnosed, in which caseradiopaque calculus could be identified. Conclusion:CBCT technology can be widely used in dental care for diagnosis and clinical treatment. It is helpful for the dentist to make a diagnosis, design a treatment plan, and to avoid risks of thesurgery. Furthermore, it can give adjunctive information to improve the diagnosis and treatment efficacy, and thus is worthy of further application.
