摘 要:目的在体内微环境中研究局部应用苯妥英钠对大鼠牙周膜干细胞迁移及成骨分化的影响。方法 54只SD雄性大鼠建立下颌骨创伤模型,创伤局部使用预定浓度的苯妥英钠,将大鼠随机分为实验组和对照组。实验组于术后通过舌下静脉注入BrdU标记的大鼠牙周膜干细胞,对照组于术后通过舌下静脉注入等量的PBS,每组每个浓度分别于术后1、2、4周时腹腔麻醉后脱颈处死大鼠3只,采用免疫组织化学染色和图像分析软件测定创伤区BrdU、骨桥蛋白(OPN)、RUNX2的表达,并用SPSS 18.0统计分析软件进行多因素方差分析。结果 40μg/mL PHT+rPDLSCs组和100μg/mL PHT+rPDLSCs组BrdU阳性细胞数明显多于20μg/mL PHT+rPDLSCs组与rPDLSCs,差异有统计学意义(P<0.05);40μg/mL和100μg/mL PHT+rPDLSCs与rPDLSCs相比、PHT和空白组相比、PHT+rPDLSC与PHT相比差异有统计学意义(P<0.05);40μg/mL和100μg/mL PHT组OPN和RUNX2的表达差异无统计学意义(P>0.05)。结论 40μg/mL和100μg/mL PHT在骨组织修复的早期可以促进缺损附近的牙周膜干细胞迁移至缺损区域,并增殖分化为成骨细胞,促进骨组织的愈合。Objective To explore the effects of local application of phenytoin(PHT)on the rat periodontal ligament stem migration and differentiation in vivo microenvironment.Methods We established a mandibular acute defect model in 54 SD male rats.The defect was treated with PHT of different concentrations.After surgery the experimental group received injection of 5-bromodeoxyuridine(BrdU)-labeled rat periodontal ligament stem cells(rPDLSCs)while the control group was injected with the same amount of PBS.The rats were sacrificed by 3 at 1 w,2 w and 4 w for HE staining and immunohistochemical staining of BrdU,OPN and RUNX2 using image Proplus image analysis and statistical analysis.Results PHT of 40μg/mL and100μg/mL promoted the formation of bone on the surface of defects.Immunohistochemical staining showed that the number of BrdU staining on PHT side was greater than in the control group while RUNX2 and OPN expressions did not differ significantly in 40μg/mL and100μg/mL PHT staining groups(P〈0.05).Conclusion PHT of 40μg/mL and100μg/mL can make periodontal ligament stem cells migrate to the defect area,proliferate or differentiate into osteoblasts,thus promoting healing of bone tissues.
