摘　　要：目的腹腔镜肝脏切除术近年来已经取得了迅速发展,但是腹腔镜右半肝切除仍被普遍认为是一项高难度、高风险手术,未广泛推广和开展。解放军总医院肿瘤外二科团队总结了模式化右半肝切除术的方法,并将其进行了初步应用。方法回顾性分析2012年7月至2014年12月于解放军总医院肝胆胰肿瘤外科行模式化腹腔镜下右半肝切除术的患者资料,分析其手术时间、术中出血量、术后住院时间等指标。结果共入选23例患者,其中男性13例,女性患者10例,平均年龄(52.96±11.37)岁,肿瘤平均直径(9.27±4.96)cm,平均手术时间(256.17±71.16)min,平均出血量(493.48±239.94)ml,术后平均住院时间(8.91±3.6)d。1例术后出现胆瘘,1例出现切口感染,其他患者恢复顺利,无术后并发症。结论模式化右半肝切除术安全可行,对腹腔镜右半肝的推广可起到积极作用。Objective Laparoscopic liver resection has made rapid development in recent years, However laparoscopic right hemihepatectomy is stillregarded as a complicated and high-risk surgery, and yet has not been widely applied. Our team sμmmarizes the stylized laparoscopic right hemihepatectomy method, and has carried on the preliminary application. Methods We retrospectively analyzed the patients data from Jul. 2012 to Dec. 2014, who underwent laparoscopic right hemihepatectomy in department of Department 2 of Oncology Surgery of PLA general hospital, including operation time, intraoperative blood loss, postoperative length of hospital stay. Results Total of 23 patients enrolled, There were 13 males and 10 females, cases of male, average age of （52.96±11.37）, average tμmor diameter （9.27 ±4.96） cm, average operation time （256. 17± 71. 16） rain, average blood loss （493.48 ± 239.94） ml, average postoperative length of hospital stay was （8.91± 3.6 ） days. Postoperative complications occurred in 2 cases, including 1 biliary leakage and 1 incision infection. The rest of the patients recovery smoothly, without postoperative complications. Conclutions Stylized laparoscopic right hemihepatectomy is safe and feasible. Stylized laparoscopic right hemihepatectomy technique will play a positive role for the promotion of laparoscopic right hemihepatectomy.
