带蒂岛状皮瓣修复外耳道肿物切除后耳周缺损

摘　　要：目的探讨带血管蒂岛状皮瓣移植在广基底型外耳道良性肿物切除术后耳周创面修复中的临床效果。方法选择2012年1月-2013年12月我院收治的100例（140耳）外耳道良性肿物患者,分为切除手术加皮瓣移植组（A组,70耳）和单纯手术切除组（B组,70耳）,每组再根据外耳道肿物切除后创面分为两个亚组,每组35耳：A1组和B1组,创面半周以下,A2组和B2组,创面半周以上。A1组和A2组采用带血管蒂岛状皮瓣移植修复缺损,B1组和B2组患者未采用皮瓣移植,自行修复外耳道缺损。结果所有患者随访1.2-2.5年。组间外耳道缺损上皮化时间及治愈率比较,A1组与B1组、A2组与B2组间差异均有统计学意义（P〈0.05）。皮瓣移植患者移植皮肤的质地与移植前基本相似,皮肤色泽均与正常皮肤无区别。结论皮瓣移植修复外耳道良性肿物切除术后耳周创面缺损效果可靠,术后愈合时间缩短。Objective To investigate the clinical effect of pedicled island flap for repairing the skin defect due to goiter resection of external auditory canal. Methods A total of 100 patients（ 140 ears involved） with external auditory canal goiter in our hospital from January 2012 to December 2014 were reviewed. All 100 cases were divided into group A and B according to surgical procedures,following were divided into two subgroup for each group： Group A1 with circumferential defect below hemicycle and group A2 with circumferential defect above hemicycle,both subgroups were received goiter resection plus pedicled island flap repair; Group B1 with circumferential defect below hemicycle and group B2 with circumferential defect above hemicycle,both subgroups were carried out single goiter resection. Results All patients were followed up for 1. 5 to 2 years. There was statistic difference in complete epithelization time and recovery rate between group A1 and B1,group A2 and B2（ P〈 0. 05） respectively. The texture and color of grafting skin was normal in both subgroup A1 and A2. Conclusion The skin flap can achieve ideal effect with shot recovery duration for repairing the skin defect after goiter resection of external auditory canal.
