摘 要:目的探讨激光引导泊机装置在机器人腹腔镜前列腺癌根治术的术前准备中的临床应用价值。方法2013年7月至2014年6月,单术者完成机器人前列腺癌根治术40例,其中20例采用传统方法进行泊机(10例由新护士泊机,另10例由操作机器人系统1年以上的护士泊机),另20例采用激光引导泊机装置泊机(10例由新护士泊机,另10例由操作机器人系统1年以上的护士泊机),分别记录泊机时间(泊机起点与终点位置均统一固定)和调整角度次数。结果手术均顺利安全完成,无中转开放。采用传统方法泊机过程中,新护士所需时间为225-272秒,平均时间为(254±18)秒,调节角度次数为7.0~12.0次,平均次数为9.2±3.4次,操作机器人系统1年以上的护士所需时间为30~45秒,平均为(39±7)秒,调整角度次数为0.0~2.0次,平均(1.1±1.0)次;采用激光引导泊机装置泊机过程中,新护士所需时间为45~72秒,平均时间为(60±14)秒,调节角度次数为3.0~5.0次,平均次数为(4.2±1.3)次,操作机器人系统1年以上的护士所需时间为25~37秒,平均为(31±5)秒,调整角度次数为0.0~2.0次,平均(1.1±1.0)次。结论采用激光引导泊机装置泊机,明显缩短了新手泊机时间,方便操作,利于初学者短时间内高效完成泊机环节。To explore the clinical significance of laser guiding system used in the preparation for the robotic-assisted radical prostatectomy. methods 40 cases of robotic-assisted radical prostatectomy were performed between July 2013 and June 2014, 20 cases were performed by traditional ways to dock the robot (10 cases were performed by the new nurse, others were done by experienced nurse who had been trained over one year on the robot), other 20 cases were performed by the laser guiding system (10 cases were performed by the new nurse, others were done by experienced nurse who had been trained over one year on the robot). Keep a record of the time of docking the robot and the times of adjusting the angle.Resuit All the cases of robotic-assisted radical prostatectomy were performed safely without conversions to open surgery. Using the traditional way, the new nurse needed 225-272 senconds, the average time was (254± 18) senconds, the times of adjusting the angle were 7.0±12.0, the average times were (9.2±3.4). For the experienced nurse, the time of docking robot were 30-45 seconds, the average was (39_+7) seconds, the times of adjusting the angle were 0.0-2.0, the average of (1.1±1.0). However, using the laser guiding system to dock the robot, the new nurse just needed 45-72 seconds, the average time was (60±14) seconds, the times of adjusting the angle were 3.0-5.0 times, the average was (4.2 ±1.3). The experienced nurse needed 25-37 seconds, the average (31±5) seconds, the time of docking robot were 0.0-2.0, with an average of(1.1 ±1.0). Conclmsion Using the laser guiding system can significantly shorten the time for docking the robot. It is easy to manipulate and is helpful for novice to dock the robot efficiently.
