加味白头翁汤治疗溃疡性结肠炎的临床疗效分析

摘　　要：目的:分析加味白头翁汤治疗溃疡性结肠炎的临床疗效。方法:将医院收治的78例溃疡性结肠炎患者随机分为观察组39例和对照组39例。对照组患者行西医常规治疗,观察组患者在西医常规治疗的基础上行加味白头翁汤治疗。评价两组患者的临床治疗效果,对患者治疗前后的中医证候积分进行评价,并对患者治疗前后的血清炎性指标内毒素、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)进行检测。结果:观察组患者临床治疗的总有效率为92.32%,对照组为76.93%,两组比较,差异具有统计学意义(P<0.05)。治疗后,两组患者的各中医证候积分较治疗前均下降(P<0.05),但观察组患者腹泻、腹胀、腹痛、脓血便、里急后重中医证候积分的下降幅度均大于对照组(P<0.05)。治疗后,两组患者的平均血清内毒素、IL-1β、TNF-α指标值较治疗前均下降(P<0.05),但观察组患者各指标的下降幅度均大于对照组(P<0.05)。结论:在西医常规治疗的基础上联合应用加味白头翁汤能有效提高溃疡性结肠炎患者机体炎性反应的改善效果,增强结肠黏膜的修复效果,进而提高临床整体疗效,值得推广应用。Objective： To analyze the clinical curative effect of Jiawei Baitouweng Decoction in treating ulcerative colitis. Methods： 78 cases of ulcerative colitis patients in our hospital were randomly divided into the observation group（ 39cases） and the control group（ 39 cases）. The control group was treated by routine western medicine and the observation group was treated by Baitouweng Decoction based on the routine treatment. The clinical therapeutic effect of 2 groups were evaluated. Before and after treatment,the TCM syndromes were evaluated and the serum inflammatory markers in patients with endotoxin,interleukin- 1 beta（ IL- 1 β）,tumor necrosis factor alpha（ TNF- α） were detected. Results： The observation group＇s total effective rate was 92. 32% and the control group＇s was 76. 93% and there was statistically significant difference（ P 〈0. 05）. After treatment,2 groups different TCM syndromes scores were decreased（ P 〈0. 05）,but the observation group＇s diarrhea,abdominal distension,abdominal pain,purulent blood,tenesmus TCM syndrome integral declines were greater than the control group＇s（ P 〈0. 05）. The average after treatment of two groups＇ serum endotoxin,IL- 1 β and TNF- α indexesdecreased（ P 〈0. 05）,but the index decline of observation group was greater than the control group＇s（ P 〈0. 05）. Conclusion： On the basis of conventional western medicine treatment,combined use of Jiawei Baitouweng Decoction can effectively improve the ulcerative colitis patients ＇ body inflammatory response,enhance the repair effect of colonic mucosaand improve the overall clinical curative effect,worthy of popularization and application.
