83例妊娠合并重性精神疾病患者的回顾分析

摘　　要：目的探讨患有重性精神疾病的孕产妇在妊娠期服用精神药物对新生儿、孕产妇病情变化、以及妊娠结局的影响。方法回顾性分析2012年1月-2015年1月在湖南省第二人民医院住院的83例合并有重性精神疾病的孕产妇临床资料。结果 1新生儿均无畸形,生长发育状况良好。2妊娠前6个月及孕期未服用精神类药物24例(28.91%),妊娠前6个月及妊娠期均在服用精神药物59例(71.08%);妊娠期精神疾病加重或复发33例(39.75%),产后复发23例(27.71%);病情稳定者27例(32.53%)。3足月妊娠62例(74.69%),早产2例(2.40%),人工引产16例(19.27%),自然流产3例(3.61%);自然分娩27例(42.18%),剖宫产37例(55.22%),其中脐带绕颈1例,产时胎儿轻度窒息2例。结论合并有重性精神疾病的孕产妇在妊娠中服药维持治疗可减少病情复发或加重;精神药物可能有导致流产的风险。Objective To investigate the severe mental illness maternal effects in pregnancy taking psychotropic drugs in pregnancy outcome and changes of their condition. Methods Retrospective analysis of the clinical data of the pregnant women in 2012 January- 2015 year in January had a serious mental illness patients in the 83people＇s Hospital of Hunan Province Hospital of the second. Results The newborns had no malformations,growth and development in good condition. The first 6 months of pregnancy and during pregnancy without use of psychotropic drugs in 24 cases（ 28. 91%）,the first 6 months of pregnancy and pregnancy were taking psychotropic drugs in 59cases（ 71. 08%）; pregnancy induced severe mental illness aggravation or recurrence in 33 cases（ 39. 75%）,postpartum recurrence in 23 cases（ 27. 71%）; 27 patients had stable disease who（ 32. 53%）. 62 cases of full- term pregnancy（ 74. 69%）,2 cases of premature delivery（ 2. 40%）,16 cases of induced labor（ 19. 27%）,3 cases of spontaneous abortion（ 3. 61%）; natural childbirth in 27 cases（ 42. 18%）, 37 cases of cesarean section（ 55. 22%）,including 1 cases of umbilical cord around the neck,intrapartum fetal asphyxia was mild in 2 cases.Conclusion Maternal with serious mental illness should maintain treatment to avoid relapse or aggravation in pregnancy; psychotropic drugs has led to risk of miscarriage,premature delivery.
