胶原蛋白肽防治老年骨关节炎和骨质疏松症的研究进展

摘　　要：本文以关键词胶原蛋白肽、胶原蛋白水解物、骨关节炎和骨质疏松症,检索2015年4月以前Pub Med医学资料库的体外、体内试验和临床研究文献,综述胶原蛋白肽和胶原蛋白水解物预防和治疗骨关节炎和骨质疏松症的作用进展。胶原蛋白肽早已应用于药品和食品领域,被管理机构认为一般是安全的食品产品;胶原蛋白肽(如Peptan)是由健康的猪、牛和鱼的骨骼和皮肤水解制得;体外实验和体内试验显示,胶原蛋白肽能刺激关节软骨产生Ⅱ型胶原蛋白、透明质酸、黏多糖和蛋白聚糖,抑制关节软骨破坏,增加成骨细胞介导的骨形成,减少破骨细胞介导的骨吸收,以及增加骨密度;临床研究表明,膝关节骨关节炎患者每日口服Peptan胶原蛋白肽8 g共6个月可以显著减少关节疼痛和改善关节功能,胶原蛋白肽可以增加血的羟脯氨酸浓度,减少绝经后骨质疏松症妇女的骨质流失,并加强和延长抗骨质疏松药物治疗的效果。胶原蛋白肽作为营养素具有预防和治疗骨关节炎和骨质疏松症的潜在效用。Using collagen peptides, collagen hydrolyzate, osteoarthritis and osteoporosis as keywords, authors searched medical database ofin vitro andin vivo experiments and clinical studies literatures from PubMed before April 2015, and reviewed effects of collagen peptides and collagen hydrolyzate on the prevention and treatment of osteoarthritis and osteoporosis. The results showed that collagen peptides have long been used in pharmaceuticals and foods, which are generally recognized as safe food products by regulatory agencies; collagen peptides (such as Peptan) is obtained by hydrolysis of bone and skin of healthy pig, bovine and fish. Trialsin vitro andin vivo have shown that collagen peptides stimulate the articular cartilage to produce typeⅡ collagen, hyaluronic acid, glucosaminoglycan and proteoglycans, inhibit cartilage damage, increase osteoblast-mediated bone formation, reduce osteoclast-mediated bone resorption and increase bone density. Clinical studies suggested that the ingestion of 8 g Peptan collagen peptides daily for 6 months can significantly reduce joint pain and improve joint function in patients with osteoarthritis of the knee; Collagen peptides can increase blood concentration of hydroxyproline, reduce post-menopausal women with osteoporosis bone loss and strengthen and extend the anti-osteoporosis drug treatment.In conclusion, collagen peptides are of interest as a nutraceutical of potential utility in the prevention and treatment of osteoarthritis and osteoporosis.
