摘  要:目的探讨云南省昆明市妇幼保健院新生儿疾病筛查中心实验室先天性甲状腺功能减低症(CH)筛查切值,以便能降低漏诊率和假阳性率。方法新生儿于出生72 h后采集足跟血,采用荧光酶免疫分析方法检测滤纸干血片促甲状腺激素(TSH)浓度。结果通过检测2010-2014年共119 718例新生儿足跟血标本,检出正常新生儿数为119 657例,确诊CH患儿42例,高TSH血症患儿19例,检出率为1∶1 963。根据筛查结果,确定云南省昆明市妇幼保健院新生儿疾病筛查中心实验室荧光酶免疫法筛查CH参考切值为10.0 m U/L,其灵敏度达100.00%,特异度97.73%,漏诊率为0.00%。结论新生儿TSH筛查切值的确立为云南省昆明市妇幼保健院新生儿疾病筛查中心实验室今后新生儿CH的筛查提供依据。Objective To explore the screening cut-off value of congenital hypothyroidism (CH) in laboratory of Screening Center of Neonatal Diseases in the hospital, reduce the missed diagnostic rate and false positive rate. Methods Heel blood samples were collected from the neonates born after 72 hours, fluorescent enzyme immunoassay was used to detect the levels of thyroid-stimulating hormone (TSH). Results From 2010 to 2014, 119 718 neonates were detected totally, 119 657 neonates were normal, 42 neonates were diagnosed as CH definitely, 19 neonates were found with high TSH level, the detection rate was 1 : 1 963. According to the screening resuhs, cut-off value of TSH was 10. 0 mU/L, the sensitivity, specificity, and missed diagnostic rate were 100. 00% , 97.73% , and 0.00% , respectively. Con- clusion The determination of TSH cut-off value of neonates provides a basis for screening of CH among the neonates in Screening Center of Neonatal Diseases in Kunming Maternal and Child tiealth Care Hospital.
