摘  要:目的探讨医学模拟教学(SBME)在中心静脉穿刺置管多媒体教学中的效果。方法选择2013年10月和2014年10月在皖南医学院参加危重病医学学习的麻醉学专业五年制2010级和2011级本科学生各30名作为教学对象,分别将2010级、2011级学生设为对照组(多媒体教学)和试验组(多媒体教学结合SBME方法)。课程结束后,对两组学生分别进行理论知识、操作技能和临床综合处理能力考核及问卷调查,评估教学效果。数据采用SPSS 17.0软件进行统计分析。结果两组学生在年龄、性别、平时专业理论成绩、理论考核成绩等方面比较,差异无统计学意义(P>0.05)。试验组临床操作技能考核成绩优于对照组,差异有统计学意义(P<0.05);临床综合处理能力考核中,两组在体格检查、临床诊断和治疗方案方面比较,差异无统计学意义(P>0.05),而在病史采集、医德医风、医患沟通、整体评价方面比较,试验组成绩均明显优于对照组,差异有统计学意义(均P<0.05);两组在学习兴趣方面比较,试验组能明显调动学生学习积极性、活跃课堂气氛(P<0.05),但没有明显促进课前预习(P>0.05);两组在培养学生自学能力和主动思维的学习能力方面比较,差异无统计学意义(P>0.05);两组在培养学生团队精神和合作能力的团队协作方面比较,试验组均优于对照组(P<0.05)。结论医学模拟教学能够显著提高中心静脉穿刺置管多媒体教学中学生的临床操作技能、团队协作和临床综合处理能力。Objective To evaluate the effectiveness of applying simulation based medical education(SBME) in central venous catheterization multimedia teaching.Methods 60 anesthesiology under-graduates in Wannan Medical College,who participated in critical care medicine teaching from 2013 to 2014 were observed.All students were divided into two groups:control group(conventional teaching,2010 grade,n=30) and SBME group(2011 grade,n=30).The teaching effectiveness was evaluated by questionnaire survey,theoretical exam,procedural skills and clinical comprehensive abilities.Data were analyzed by SPSS 17.0 software.Results There was no significant difference in essential characteristics,including age,gender,the scores of specialized theory at routine examination and the scores of theory examination between two groups(P〈0.05).The procedural skills in SBME group were significant higher than control group(P〈0.05).There was no significant difference in the physical examination,clinical diagnosis,and treatment procedures between two groups(P〈0.05).However,SBME group performed better in medical history taking,medical ethics,patient communication,and overall clinical competence than control group(P〈0.05).Conclusion SBME can improve the students' basic clinical skills,team work spirit,and comprehensive clinical capacity in the central venous catherization multimedia teaching.
