摘 要:目的了解老年人群多不饱和脂肪酸(polyunsaturated fatty acids,PUFA)水平,分析多不饱和脂肪酸与免疫炎症的相关性。方法选取中国人民解放军总医院2014年健康查体>70岁老年人168例,通过高效气相色谱毛细管层析法(HPLCMS)检测标本中ω-3 PUFA和ω-6 PUFA的含量及构成,并分别检测淋巴细胞亚群、炎性细胞因子和C反应蛋白,分析多不饱和脂肪酸与免疫炎症的相关性。结果本组人群总ω-3 PUFA 5.68%±2.01%,二十碳五烯酸(eicosapentaenoic acid,EPA)0.32%±0.21%,二十二碳六烯酸(docosahexaenoic acid,DHA)3.27%±1.10%,ω-3指数(ω-3 index)3.59%±1.18%;总ω-6 PUFA 26.76%±4.56%,ω-6/ω-3比值5.56±2.77。EPA、DHA、ω-3 index与IL-2呈负相关(rEPA=-0.153 2,rDHA=-0.305 5,rω-3 index=-0.319 5,P<0.05),EPA、ω-3 index与PGE2呈负相关(rEPA=-0.167 8,rω-3 index=-0.245 8,P<0.05),花生四烯酸与COX-2呈负相关(rAA=-0.167 7,P<0.05),PUFA与淋巴细胞亚群无相关性(P>0.05)。结论本组老年人群ω-3多不饱和脂肪酸处于缺乏水平,ω-3多不饱和脂肪酸对机体有抗炎作用。Objective To investigate the level and structure of polyunsaturated fatty acid(PUFA) of Chinese elderly population and analyze the relationship between the level of polyunsaturated fatty acids and inflammatory and immune biomarkers. Methods One hundred and sixty-eight elderly people(more than 70 years old) who had undergone health examination in Chinese PLA General Hospital in 2014 were selected. The content and construction of ω-3 PUFA and ω-6 PUFA in blood samples were detected using high performance liquid chromatography-mass spectrometry(HPLC-MS). The lymphocyte subsets, inflammatory cytokines and the level of C-reaction protein(CRP) were detected and the relationship between polyunsaturated fatty acid and immune inflammation was analyzed. Results The level of ω-3 PUFA, EPA, DHA and ω-3 index was 5.68%±2.01%, 0.32%±0.21%, 3.27%±1.10% and 3.59%±1.18%, respectively. The total ω-6 PUFA was 26.76%±4.56%, and the ratio of ω-6 and ω-3 was 5.56±2.77. IL-2 was negatively correlated with EPA, DHA and ω-3 index(rEPA=-0.153 2, rDHA=-0.305 5, rω-3 index =-0.319 5, P〈0.05), PGE2 was negatively correlated with EPA and ω-3 index(rEPA=-0.167 8, rω-3 index =-0.245 8, P〈0.05), and COX-2 was negatively correlated with AA(rAA=-0.167 7, P〈0.05). While, fatty acids were not correlated with lymphocyte subsets. Conclusion The elderly population enrolled in this study is lacking of ω-3 PUFA which has anti-inflammatory effects to our body.
