n-3多不饱和脂肪酸对2型糖尿病患者胰岛素抵抗及血糖的影响  被引量:6

摘  要:【摘要】目的探讨膳食n-3多不饱和脂肪酸对2型糖尿病患者血糖及胰岛素抵抗的影响。方法2型糖尿病患者5l例,随机分为三组:金枪鱼油组(n=21),亚麻籽油组(H=16)和玉米油组(n=14),三组患者分别口服相应胶囊,3种胶囊重量相等,为期180d。试验前1d及第90、180天,测定所有患者空腹血糖、胰岛素、血浆磷脂脂肪酸及其他常规血液指标。结果金枪鱼油组第180天空腹血糖(FBS)、空腹胰岛素(FINS)、糖化血红蛋白(HbAlC)、胰岛素抵抗指数(HOMA—IR)低于亚麻籽油组及玉米油组,差异有统计学意义;金枪鱼油组第180天测定血浆磷脂脂肪酸20:5n-3、22:5n-3、22:6n-3、总n-3PUFA、n-3:n-6含量高于玉米油组,20:5n.3、22:6n.3、总n-3PuFA、n-3:n-6高于亚麻籽油组,差异均有统计学意义;2型糖尿病患者的胰岛素抵抗水平和血浆中20:5n.3(r=-0.517,P〈0.01)、22:5n.3(r=.0.385,P〈0.05)、22:6n-3(r=-0.436,P〈0.01)呈负相关。结论2型糖尿病患者增加n-3多不饱和脂肪酸摄入,尤其是富含EPA与DHA的鱼油,有益于控制血糖及减轻胰岛素抵抗。Objectives The study is to investigate the effect of dietary n-3 polyunsaturated fatty acids on blood glucose and insulin resistance in Chinese type 2 diabetic patients. Methods 51 Chinese T2D patients were recruited, and all of them were randomized into three groups: tuna oil ( n=21, 3g/d/head ) , flaxseed oil ( n=16, 3g/d/head ) and corn oil ( n=14, 3g/d/head ) . Three groups had no no statistical difference in the age, sex, BMI and biochemical indexes .The intervention was last for 180 days. The fasting blood glucose, insulin, plasma phospholipid fatty acid and other routine blood indexes of all patents were measured on the zeroth day, the 90th day and the 180th day. Results Tuna oil group on the 180th day was significantly lower than Flaxseed oil and corn oil group in fasting blood glucose ( FBS ) , fasting insulin ( fins ) , glycosylated hemoglobin ( HbAlc ) , insulin resistance index ( HOMA-IR ) , the difference is statistically significant.Tuna oil group on the 180th day was significantly higher than corn oil group in plasma phospholipid fatty acid 20: 5n-3, 22: 5n-3, 22: 6n-3, total n-3 PUFA, and n-3:n-6 contents, and it was significantly higher than flaxseed oil group in 20: 5n-3, 22: 6n-3, total n-3 PUFA, and n-3:n-6 contents.Insulin resistance levels of T2D patints is negative correlated to 20:5n-3 ( r=-0.517, P〈0.01 ) and 22:5n-3 ( r=-0.385, P〈0.05 ) , 22:6n-3 ( r=-0.436, P〈0.01 ) contents. Conclusion Increasing the intake of n-3 polyunsaturated fatty acid, especially fish oil rich in EPA and DHA is beneficial to control blood glucose and reduce insulin resistance.
