香菇多糖联合顺铂胸腔注药治疗肺癌合并恶性胸腔积液的meta分析

摘　　要：目的系统评价香菇多糖联合顺铂胸腔注药治疗肺癌合并恶性胸腔积液的有效性及安全性。方法计算机检索Pub Med、中国期刊全文数据库、万方数字化期刊全文数据库、中文科技期刊全文数据库从建库至2015年10月发表的香菇多糖联合顺铂胸腔注药治疗肺癌合并恶性胸腔积液的随机对照研究(RCT)文献,对纳入文献质量进行分析后,采用Rev Man 5.3软件进行meta分析。结果纳入10个RCT,共621例患者。meta分析结果显示,与顺铂组相比,香菇多糖联合顺铂胸腔注药(联合组)可显著提高肺癌合并恶性胸腔积液患者的总有效率和生活质量改善率(P<0.01),不显著增加胃肠道反应发生率和白细胞减少发生率(P>0.05)。结论香菇多糖联合顺铂胸腔注药治疗肺癌合并恶性胸腔积液临床疗效好,安全性高。Objective To systematically review the effect and safety of Lentinan combined with Cisplatin in intrapleural injection of lung cancer patients with malignant pleural effusion. Methods The randomized controlled trials（ RCTs） about Lentinan combined with Cisplatin in intrapleural injection of lung cancer patients with malignant pleural effusion were searched from Pub Med,CNKI,Wan Fang and VIP data databases during the databases establishment and October 2015. The meta-analysis was performed using Rev Man 5. 3 software after analyzing quality of the included studies.Results A total of 10 RCTs involving 621 patients were finally included. The meta-analysis showed that the total effective rate and improvement rate of life quality were significantly improved（ P〈0. 01）,but the incidence rates of gastrointestinal reactions and leucopenia were not significantly increased by Lentinan combined with Cisplatin compared with those by Cisplatin only in treatment of lung cancer patients with malignant pleural effusion（ P〉0. 05）. Conclusion Lentinan combined with Cisplatin in intrapleural injection of lung cancer patients with malignant pleural effusion has good curative effect and safety.
