摘  要:目的探讨营养状况及心理因素对胃癌患者半年生存率及生存质量的影响。方法选择2013年1月~2015年3月术后胃癌患者87例为研究对象,术后进行第一次调查,术后随访半年进行第二次调查,分析患者营养、心理因素对生存率、生存质量的影响。结果术后随访半年,76例存活,存活率87.4%,死亡率12.6%。87例患者术后14 d调查,营养状况综合因子得分:好22例(25.3%),中43例(49.4%),差22例(25.3%)。87例患者中,无焦虑21例,可疑焦虑18例,肯定焦虑48例。无抑郁28例,可疑抑郁15例,肯定抑郁44例。28例采用面对方式,24例采用回避方式,35例采用屈服方式。随访半年,生存时间〈6个月组患者营养状况更差、肯定焦虑的比例更高,肯定抑郁的比例更高,应对方式以屈服的比例更高(P〈0.05或P〈0.01)。随访半年,生存质量好者营养状况更好,无焦虑比例更高,无抑郁比例更高,应对方式以面对的比例更高(P〈0.05或P〈0.01)。结论营养状况差、焦虑、抑郁、消极应对的胃癌患者术后生存时间相对较短,生存质量较差。Objective To discuss the influence of nutritional status and psychological factors on half-year survival rate and quality of life in patients with gastric carcinoma. Methods 87 postoperative patients with gastric carcinoma from January 2013 to March 2015 were selected and surveyed for the first time after operation and for the second time at half a year after operation. The influence of nutritional status and psychological factors on survival rate and quality of life was analyzed. Results At half a year after operation, 76 patients survived(survival rate 87.4%, and mortality rate 12.6%). Of 87 patients at 14 days after operation, the nutritional status of 22 patients was good(25.3%), of 43 patients was moderate(49.4%), and of 22 patients was poor(25.3%). 21 patients were free of anxiety, 18 patients had suspicious anxiety, and 48 patients had confirmed anxiety. 28 patients were free of depression, 15 patients had suspicious depression, and 44 patients had confirmed depression. 28 patients adopted facing pattern, 24 patients adopted dodging pattern, and 35 patients adopted succumbing pattern. At half a year after operation, patients surviving less than 6 months showed poorer nutritional status, higher proportion of confirmed anxiety and depression, and higher proportion of succumbing pattern(P〈0.05 or P〈0.01); patients with higher quality of life had better nutritional status, higher proportion of free of anxiety and depression, and higher proportion of facing pattern(P 0.05 or P 0.01). Conclusion Patients with poor nutritional status, anxiety, depression, and negative attitude have shorter survival time and poor quality of life.
