摘 要:目的了解浙江省宁波市人乳头瘤病毒16型(Human Papillomavirus Type16,HPV16)的分布和L1区基因变异情况。方法收集宁波市社区和某医院就诊妇女的宫颈脱落细胞样本进行HPV分型检测,并对其中HPV16阳性株进行基因序列分析,并预测蛋白疏水性及抗原性。结果在778例社区人群样本中HPV16阳性23例,检出率3.0%,在1 848例就诊人群样本中HPV16阳性87例,检出率4.7%;在HPV16阳性标本中随机抽取29例进行L1基因扩增,阳性率为55.2%(16/29),经测序获得完整序列13条,L1区有12处基因变异,其中C6240G、A6432G、6902插入ATC和6950缺失TGA为主要突变形式(13/13);系统进化树分析显示,宁波市HPV16存在亚洲型和欧洲型;和参考株相比,其蛋白疏水性和抗原性在氨基酸289~292位减弱、448~449位增强、450~465位减弱。结论宁波社区人群HPV16感染率较高,多数感染者携带病毒的L1基因发生变异,这些突变引起HPV16 L1蛋白疏水性和抗原性改变。Objective To identify the distribution of human papillomavirus type 16(HPV16) and analyze the mutation in L1 region of HPV16 in Ningbo city,Zhejiang province.Methods Exfoliated cervical cells were collected from women in communities and a hospital in Ningbo and were tested for HPV subtypes.The L1 genes of HPV16 strains were sequenced and analyzed for prediction of the protein hydrophobicity and antigenicity.Results 23(3.0%) of 778 samples from communities and 87(4.7%) of1 848 samples from hospitals,respectively,were HPV16 positive.16(55.2%) of 29 samples randomly selected from these positive samples were positive for the L1 gene of HPV16 as determined by amplification and sequencing.Thirteen complete sequences were obtained.Mutations were found in 12 sites of L1 genes,mainly C6240 G,A6432G,the insertion of ATC at site 6902,and the deletion of TGA at site 6950.The phylogenetic tree of HPV16 L1 gene showed that there were Asian and European types of HPV16 in Ningbo;compared with the reference strain,protein hydrophobicity and antigenicity of Ningbo strains was weaker at sites 289-292 and 450-465 of amino acids,and greater at sites 448-449.Conclusions The infection rate of HPV16 was relatively high in a community in Ningbo.The L1 genes of HPV16 strains among most of infected people were mutated.These mutations lead to the transformation of protein hydrophobicity and antigenicity.
