摘 要:目的:观察不同能量Er:YAG激光照射牙本质不同时间后,对髓腔温度及牙本质形态、微观结构、Ca/P比值的影响。方法:取96个离体牙制备牙合面牙本质暴露面后,随机分为A(照射10 s)、B(照射20 s)两组;每组再随机分为6个亚组,分别用Er:YAG激光以1、2、3、4、5、6 W照射,记录髓腔温度的变化。各取54个离体牙制备216个牙本质块,24个用于空白对照;192个按上述同样方法分组和照射后,用体视显微镜测量其表面凹坑的直径和深度、扫描电镜观察其表面的微观结构、色散X线荧光光谱检测其表面的Ca/P比值。结果:随激光功率的增加和照射时间的延长,髓腔温度呈上升趋势(P〈0.05),但均在安全范围;凹坑直径、深度亦逐渐增大(P〈0.05);电镜下可见,各照射组牙本质表面的玷污层消失、牙本质小管逐渐开放、管周牙本质逐渐突出,未发现牙本质熔融现象;用1~6 W不同功率照射10 s时,各功率组牙本质表面的Ca/P比与空白对照组相似(P〉0.05)。结论:本实验设定的Er:YAG能量和照射时间可安全、有效的作用于牙本质。AIM: To observe the changes of surface morphology,dental structure and the temperature of dental pulp cavity after Er: YAG laser irradiation. METHODS: 96 extracted premolars were randomly divided into2 groups and were irradiated for 10 s and 20 s with the irradiation power of 1,2,3,4,5 and 6 W,respectively. The temperature of dental pulp cavity was detected by digital thermometer. 216 dentin blocks were prepared from 56 extracted premolars,24 were served as blank controls and 192 were used for morphological assay and microscopic observation. The laser irradiation was the same as mentioned above. The diameters and depths of dentin pits were observed under light microscope and the microstructure of the dentin was observed by SEM. The Ca / P ratios were detected bydispersive X- ray fluorescence spectrometry. RESULTS: With the extension of irradiation time and increase of energy,the temperature of dental pulp cavity was increased( P〈0. 05),but still in the safe range. The diameter and depth of dentin pits were increased( P〈0. 05). After irradiation,dentin surface was rough and clean,dentinal tubule was open,the dentin around tube was prominent on the surface,dentin melting was not found. No difference in C / P ratio was observed between the irradiation and the control groups( P〉0. 05). CONCLUSION: The Er: YAG laser irradiation time and power in this experiment are safe and effective on dentin.
