《内外伤辨惑论》浅析

摘　　要：东垣先生作为"补土派"的代表人物,《内外伤辨惑论》是其第一部著作,其理论经验在其后续的《脾胃论》和《兰室秘藏》等著作中均有所发挥和完善。东垣先生对脾胃疾病的论治,在师承张元素学术思想的基础上,理论联系实际,发明"内伤"。以清晰和有序的态度着重介绍了内伤与外感发热的区分方法,内伤疾病从脾胃论治的缘由,以及脾胃与众多疾病的关系、用药宜忌和因时节病症不同用药加减的变化等方面,并在继承和发扬其师张元素的从脾胃论治疾病的理论、方法,创立了"甘温除热"思想及诸如补中益气汤、羌活胜湿汤、升阳散火汤等著名方剂,为后人治疗内伤热病提供了新的视野,并发展和推动了重视脾胃的思想及从脾胃论治疾病的方法。另外,其在《伤寒论》的基础上,发展了从脾胃论治内伤疾病的学说,注重脾胃元气,以甘温益气之药,升阳补中,从而治疗多种疾病,值得后世学习探讨。Clarifying Doubts about Damage from Internal and External Caases is the first work of Doctor Li Dong-yuan,who is the representative of ＂the School of Reinforcing the Earth＂. His theory and experiences are developed and perfected in the following works such as Treatise on the Spleen and Stomach and Secrets from the Orchid Chamber. In treating the diseases related to the spleen and stomach,he connects theory with practice and invents ＂ internal injuries＂ on the basis of Zhang Yuan-su＇s academic thought. In this book, Li especially introduces the method of telling internal injuries from exogenous fever with With clear and orderly manner, the cause of treating internal injury from the spleen and stomach, the relationship between spleen and stomach and many diseases, drug-use suggestions and taboos, the modification of drugs due to different seasons, symptoms, inherits and develops Zhang Yuan-su＇s theory and methods,establishes the thought of ＂relieving fever with sweet and warm-natured drugs＂ and many famous prescriptions such as Center-Supplementing and Qi-Boosting Decoction, Notopterygium Dampness-Drying Decoction, Yang- Raising and Fire-Removing Decoction,which provides a new horizon for treating fever due to internal injuries,develops and pro- motes the idea of paying attention to the spleen and stomach, and treating diseases from the spleen and stomach. Besides, based on Treatise on Cold Damage Diseases ,he develops the theory of ＂internal injuries＂ theory treated from the spleen and stomach,pay- ing attention to renal qi of the spleen and stomach,raising yang and nourishing the middle energizer with sweet and warm-natured drngs,thus many kinds of diseases can be cured,which is worth studying and exploring.
