摘　　要：目的探讨三维动脉自旋标记(3D-ASL)在脑梗死中的初步应用价值。方法对24例脑梗死患者分别行常规MRI、DWI及3D-ASL序列扫描,观察3D-ASL与DWI序列病变区最大层面面积大小及缺血区域与对侧镜像层面的脑血流量情况,取脑梗死患者的梗死灶最大层面,在DWI、3D-ASL-CBF图上手动勾勒梗死面积和异常灌注面积,非参数检验比较两者间的差异。结果所有脑梗死患者在DWI图上表现为大小不等的高信号,梗死面积为(617.65±1 074.50)mm2,3D-ASL-CBF图上异常灌注面积为(996.39±1687.20)mm^2,并可观察到高、低灌注状态。3D-ASL-CBF图上异常灌注面积大于DWI图上梗死灶面积,且差异有统计学意义(P=0.043)。结论 3D-ASL是一种安全无创的脑血流测量技术,可有效显示梗死灶的血流灌注改变,对于脑梗死的诊断有重要的应用价值,可全面反映脑梗死的血流灌注状态及动态观察脑梗死的血流恢复情况,并可初步判断脑梗死可存活区,有利于及时指导脑梗死溶栓治疗并判断预后。Objective To evaluate the three-dimensional arterial spin labeling（ 3D-ASL） preliminary application value in cerebral infarction. Methods Totally 24 cerebral infarction patients underwent conventional MRI,DWI and 3D-ASL scan,the lesion area and its CBF value were measured on DWI and 3D-ASL maps in the patients with cerebral infarction and mirror control area by using the manual method,simultaneously compared the difference by using nonparametric test. Results All patients with cerebral infarction showed hyperintensity on DWI maps,the infarct area was（ 617. 65 ± 1074. 50） mm2,and the abnormal area on 3D-ASL-CBF was（ 996. 39 ± 1687. 20） mm2,and high and low perfusion were displayed in 3D-ASL-CBF maps. The abnormal area on 3D-ASL-CBF maps was significantly larger than that on the DWI maps（ P = 0. 043）. Conclusions 3D-ASL is a safe,noninvasive brain blood flow measurement technology,which can effectively show the blood perfusion of infarcts change,and has important application value in the diagnosis of cerebral infarction,can comprehensively reflect the state of the blood perfusion of cerebral infarction and dynamic observation of blood flow to the brain infarction recovery,the survival area of the cerebral infarction can be preliminarily judged,which is beneficial to guide the treatment of cerebral infarction and to judge the prognosis of cerebral infarction in time.
