摘  要:目的比较两种不同参考面下角膜后表面高度的差异,为临床应用提供指导。方法前瞻性临床研究。选取2014年8月至2015年8月在武汉爱尔眼科医院汉口医院屈光手术中心就诊的近视患者40例80只眼(男18例,女22例),平均年龄(22.5±4.8)岁(18~35岁),平均等效球镜(-4.84±2.26)D(-1.00~9.75D)。使用Pentacam HR眼前节分析仪测量角膜后表面高度,分别以最佳拟合球面、最佳拟合环曲面椭球面为参考面,记录两种参考面下角膜顶点、角膜最薄点以及5mm直径范围最大的角膜后表面高度值。采用配对f检验比较两种参考面下角膜后表面高度的差异,Pearson相关分析两种参考面下角膜后表面高度的相关性。结果以最佳拟合球面为参考面的角膜顶点、角膜最薄点和5mm直径范围最大的角膜后表面高度平均值分别为(1.7±2.4)μm、(4.3±3.4)μm和(10.8±3.1)μm,以最佳拟合环曲面椭球面为参考面的平均值分别为(-3.0±1.2)μm、(-1.6±1.8)μm和(7.0±4.0)μm。以最佳拟合环曲面椭球面为参考面的角膜后表面高度值均低于最佳拟合球面,差异均有统计学意义(P〈0.05),相关系数分别为0.054、-0.238、0.304,(P=0.764,0.175,0.080)。结论相较于最佳拟合球面,采用最佳拟合环曲面椭球面进行拟合能够更加贴合真实的角膜形态,可能是更佳的术前筛查角膜扩张性病变的选择。Objective To compare the difference of the posterior corneal elevation using two reference surfaces, and to provide the reference for clinical application. Methods In this prospective study, a total of 40 patients (80 eyes, 18 male, 22 female) who had preoperative examinations in the department of refractive surgery in our hospital from August 2014 to August 2015 were enrolled. Mean age of the study population was (22.5±4.8) years (ranged 18 to 35years), mean spherical equivalent was (-4.84±2.26) D (range -1.00 to -9.75 D). All eyes were measured with a Scheimpflug analyzer Pentacam HR. The posterior corneal elevation values at the corneal apex, the thinnest corneal point and the point of maximum elevation value in the central 5 mm diameter were recorded using two different reference bodies: sphere and toric ellipsoid. Paired t test was used to evaluate the difference of posterior corneal elevation values between the two reference bodies. Pearson coranalysis was used to analyze the correlation of posterior corneal elevation values using two reference bodies, Results Using the best fit sphere the posterior corneal elevation values at the corneal apex, the thinnest corneal point and the point of maximum elevation value in the central 5 mm diameter were (1.7±2.4) μm, (4.3±3.4) μm and (10.8±3.1) μm respectively; using the best fit toric ellipsoid the corresponding posterior corneal elevation values were (-3.0±1.2) μm , (-1.6±1.8) μm and (7.0±4.0) μm respectively. The posterior corneal elevation values under the best fit torie ellipsoid were statistically significant lower than the best fit sphere (All P 〈0.05). The correlation coefficient between the two reference bodies were 0.054,-0.238, 0.304 (P =0.764, 0.175, 0.080). Conclusions A toric ellipsoid reference body fit closer to the actual corneal surface shape than a sphere reference body and provide a better choice of preoperative refractive surgery screening for corneal ectasia diseases.
