摘 要:目的 构建三级医院护士长胜任力模型,为三级医院护士长的选拔、考核提供依据。方法 通过文献回顾、护理管理专家咨询编制护士长胜任力词典,选取3所三级医院的30名护士长进行行为事件访谈,结合对行政领导、科主任发放核检表构建初始模型;基于初始模型编制问卷,选取5所三级医院412名护士长进行调查,通过探索性因子分析建立模型框架,验证性因子分析、内容效度及内部一致性检验验证模型结构。结果 确定最终的模型结构包括3项一级指标(管理能力、影响能力、个人特质),20项二级指标,每项二级指标分为4个等级。探索性因子分析提取3个公因子,累计贡献率为70.214%;验证性因子分析显示模型结构较好;模型整体内容效度为0.950,各条目内容效度为0.890~1.000;总体Cronbach′s α系数为0.951。结论 建立的护士长胜任力模型科学合理,可作为护士长选拔、考核的工具,为医院制定培训计划及护士长自身职业生涯规划提供参考。Objective To establish a competency model for head nurses working in tertiary hospitals, so as to provide a b~ f0r se- lection and assessment of head nurses. Methods On the basis of literature review and expert consultation, an initial model was built up after event interviews with 30 head nurses from 3 tertiary hospitals. A questionnaire was designed on the basis of the initial model and used to survey 412 head nurses from five tertiary hospitals. Exploratory factor analysis was conducted to build the model framework. Confirmatory factor analysis, content validity and internal consistency inspection were used to test the framework. Re- sults The final model comprised of 3 first-level indicators (management ability, influencing power and personal traits) and 20 second-level indicators, which were further divided into four levels. Three factors were extracted using exploratory factor analysis, contributing 70. 214~ of the total variance. Confirmatory factor analysis revealed a good fit of the model based on various fit indice. The S-CVI was 0. 950, and I-CVI ranged from 0. 890--1. 000. The Cronbach's coeffeicient was 0. 951. Concihsion The competency model established is scientific and can be used as a tool for selecting and assessing head nurses.
