摘 要:目的研究爆破性减压复合脊髓损伤对大鼠脊髓微循环的影响。方法健康雄性sD大鼠32只,随机分为对照组、单纯脊髓挫伤组、单纯爆破减压组和爆破性减压复合脊髓挫伤组,每组8只。在伤前、伤后即刻、30、60、90 min5个时间段观察脊髓伤区及周围微循环情况,伤后6周取各组伤段脊髓,纵轴冰冻切片,HE染色,光学显微镜观察保存,借助计算机软件估算出脊髓剩余体积。结果与对照组相比,单纯脊髓损伤组、单纯爆破减压组和爆破性减压复合脊髓挫伤组评分统计差异有统计学意义(P<0.01或P<0.05),且爆破性减压复合脊髓挫伤组损伤较重。脊髓组织病理分析表明,病变持续性加重,伤后6周爆破性减压复合脊髓挫伤组平均剩余脊髓体积百分比仅为66%。结论爆破性减压可明显加重损伤脊髓微循环障碍,且有持续继发性病变加重作用,提示爆破性减压可能进一步损害脊髓,损伤大鼠的神经功能。Objective To study the effect of rapid decompression on the microcirculation of the spinal cord of rats with spinal cord injury. Methods Thirty-two male sprague-dawley four groups: control group, rapid decompression group, spinal cord injury combined with acute spinal cord injury. The residual spinal cord volumes were estimated with an optical microscope and computer software. Results Compared with the control group, there was significant difference in the scores of simple spinal cord injury group, simple burst decompression group and combined spinal cord injury group. Pathological analysis of spinal cord tissues showed that the disease duration increased. 6 weeks after injury, the percentage of the average residual spinal cord volume was 66%. Conelusion Rapid decompression can significantly and persistently aggravate the microcirculation dysfunction of the damaged spinal cord, suggesting that rapid decompression may cause more damage to the neural function of rats with spinal cord injury
