103例唐氏综合征产前诊断的临床分析

摘　　要：目的分析103例唐氏综合征胎儿的产前诊断资料,探讨唐氏综合征的产前筛查及产前诊断技术。方法收集了介入性产前诊断包括羊水穿刺及脐血穿刺的5183例孕妇资料,对其中胎儿细胞学核型为唐氏综合征的103例孕妇的年龄、产前诊断孕周、产前诊断指征和超声检查异常表现等进行分析。结果孕妇平均年龄为31.54±6.55岁,<30岁孕妇48例,占46.60%,≥30岁且<35岁者13例占12.62%,≥35岁者42例占40.78%。至少生育过一个正常孩子的孕妇有48例占46.60%,未生育的孕妇有55例占53.40%。男性胎儿71例占68.93%,女性胎儿32例31.07%。37例唐氏胎儿为唐氏筛查高风险者,占35.92%,超声提示胎儿结构异常者5例,超声软指标异常21例,两项及以上软指标异常孕妇5例,单项软指标异常孕妇16例。结论血清学筛查在产前筛查中仍然具有非常重要的意义,而不同孕期的超声筛查是对血清学筛查的重要补充。Objective：Investigate the prenatal screening and diagnosis skills of Down syndrome by analysing the clinical material of the 103 Down syndrome fetus. Methods： 103 Down syndrome fetus were detected In 5183 women who accepted the prenatal diagnosis of amniocentesis or fetal blood sampling.Analyse the clinical material of the 103 women such as ages,gestational weeks,prenatal diagnosis indiction,abnormal ultrasound appearance. Results：The average age of the 103 women is 31.54±6.59 years old,48（46.60%）women younger than 30 years old,13（12.62%）women between30 and 35 years old,42（40.78%）women older than 35 years old.There are 48（46.60%）women have at least one normal child,while 55（53.40%）women without any child.Among them71（68.93%）were male fetus and 32（31.07%）were female fetus.There are 37（35.92%）women with high-risk of Down,s screeening;5 women with abnormal ultrasound appearance;21 women with ultrasonographic soft markers while 5 with multiple soft markers,16 with Individual soft markers. Conclusions：serum screening of Down syndrome is very important in prenatal screening,and the ultrasonic examination during different pregnancy is important supplement of the serum screening.
