摘 要:目的探讨高渗氯化钠羟乙基淀粉40(HSH)对大鼠脑缺血-再灌注(I/R)损伤后神经功能行为学评分,脑水肿情况,脑梗死体积以及脑组织中基质金属蛋白酶-9(MMP-9)和层粘连蛋白(Laminin)表达的影响。方法48只雄性SD大鼠随机分为假手术组、模型组、实验1组、实验2组。大鼠局灶性脑I/R损伤模型制备成功后120 min,假手术组和模型组静脉输注0.9%生理盐水,实验1组和2组分别静脉输注HSH 4 ml/kg和8 ml/kg。再灌注24 h后,行神经功能行为学评分,观察脑水肿情况,TTC染色测算脑梗死体积,用免疫印迹方法检测缺血区脑组织MMP-9和Laminin的表达。结果与假手术组比较,模型组神经功能行为学评分升高(P<0.001);脑水肿加重;脑梗死体积增加(P<0.001);MMP-9蛋白表达升高(P<0.05);Laminin蛋白表达降低(P<0.01)。与模型组比较,实验1组和2组神经功能行为学评分降低(P<0.001);脑水肿缓解;脑梗死体积减少(P<0.001);实验1组MMP-9蛋白表达减少(P<0.01),Laminin蛋白表达升高(P<0.05);实验2组MMP-9蛋白和Laminin蛋白表达均无明显变化。结论用HSH治疗大鼠脑I/R损伤后,大鼠神经功能行为学评分改善,脑水肿减轻,脑梗死体积减小,血脑屏障通透性降低,其机制可能与降低MMP-9和增加Laminin蛋白的表达有关。且保护作用以4 ml/kg剂量效果更佳。Objective To observe the effect of Hypertonic Sodium Chloride Hydroxyethyl Starch 40( HSH) on neurological evaluations,degree of encephaledema,cerebral infarction volume and expression of MMP-9 and Laminin in rat cerebral ischemia / reperfusion( I / R) injury model. Methods 48 male SD rats were randomly divided into shame,model,experimental 1 and 2 group. Rat focal cerebral I / R injury model should been accomplished,after 120 minutes,shame and model group were treated with physiological saline,experimental 1 and 2 group were treated with 4 ml / kg and 8 ml / kg HSH by vein injection. After 24 hours' reperfusion,neurological evaluations,TTC staining and degree of encephaledema would be executed,the expression of MMP-9 and Laminin would be tested by western blotting. Results Compared to the shame group,model group's cerebral neurological evaluations( P〈0. 001),infarction volume( P〈0. 001) and expression of MMP-9( P〈0. 05) respectively increased with an obvious encephaledema,but Laminin decreased( P〈0. 05). Compared to model group,experimental 1 and 2 group's neurological evaluations( P〈0. 001) and cerebral infarction volume( P〈0. 001) decreased with a catabatic degree of encephaledema; the expression of MMP-9 in experimental 1 group decreased( P〈0. 05) and Laminin increased( P〈0. 05),but those of experimental 2 group had no obvious change. Conclusion HSH can significantly reduce neurological evaluations,cerebral infarction volume,degree of encephaledema and BBB's permeability in rat cerebral I / R injury model. These findings indicated that MMP-9 and Laminin may be involved in the protective mechanisms of HSH against cerebral I / R injury. The dose of 4 ml / kg is better.
