摘  要:目的:探讨在非小细胞肺癌组织中PD-L1、Ki67表达水平,分析其与术后进展及临床病理参数的关系。方法:应用免疫组化法检测术后肿瘤组织中PD-L1、Ki67蛋白的表达水平。结果:94例患者中PDL1阳性表达为4 4例(4 6.8 1%),阴性表达5 0例(5 3.1 9%)。Ki6 7高表达33例(35.11%),低表达61例(64.89%)。PD-L1表达与Ki67表达具有相关性(χ2=8.05,P=0.01)。PD-L1阳性表达组术后无进展(PFS)生存期为(21.86±2.93)个月,阴性表达组为(35.69±3.08)个月,两组比较具有统计学差异(χ~2=10.60,P=0.00)。Ki67高表达组术后PFS为(21.01±2.76)个月,低表达组为(33.11±2.92)个月,两组比较具有统计学差异(χ~2=5.69,P=0.02)。PD-L1表达水平与肿瘤病理类型、肿瘤分化程度及术后肿瘤进展相关(P<0.05)。Ki67表达与患者性别、病理类型及肿瘤分化程度相关(P<0.05)。多因素分析发现TNM分期、肿瘤分化程度、PD-L1对非小细胞肺癌术后PFS影响具有统计学差异(P<0.05)。结论:非小细胞肺癌中PD-L1和Ki67蛋白是预测肿瘤术后进展的重要指标,且表达越高预示术后PFS越短。Objective: To investigate the co- relationship between PD- L1 and Ki67 expression and postoperative progression in patients with non- small cell lung cancer( NSCLC). Methods: Totally 94 cases of NSCLC were collected. The PD- L1 and Ki67 expression levels were detected in cancerous tissues by using immunohistochemistry. Results: PD- L1 positive expression was in 4 4 cases( 4 6. 8 1 %) and the negative expression was in 5 0 cases( 53. 19%). The higher expression of Ki67 was in 33 cases( 35. 11 %) and lower expression was in 61 cases( 64. 89%). The PD- L1 expression levels were related to and Ki67 expression( χ2= 8. 05,P = 0. 01). The progress- free survival( PFS) of the PD- L1 positive group was( 21. 86 ± 2. 93) months,the negative group was( 35. 69 ±3. 08) months. There was a significant difference( χ^2 10. 60,P = 0. 00). The PFS of Ki67 high expression was( 21. 01 ± 2. 76) months and the group of low expression is( 33. 11 ± 2. 92) months. There was a significantly difference too( χ^2 5. 69,P = 0. 02). The level of PD- L1 expression is associated with tumor pathological type,tumor differentiation degree and postoperative progression( P〈0. 05). The Ki67 expression level is associated to gender,tumor pathological type and tumor differentiation degree( P〈0. 05). Multifactorial analysis results indicated that TNM staging,tumor differentiation degree and PD- L1 expression level are related to postoperative PFS statistically( P〈0. 05).Conclusion: The expression levels of PD- L1 and Ki67 in NSCLC tissue are related to the PFS. The more expression levels indicated that the shorter postoperative PFS.
