PHQ-2-C、CES-D和BDI-Ⅱ-C在筛查青少年抑郁障碍中的应用研究

摘　　要：目的对病人健康问卷2条目中文版(PHQ-2-C)、20个条目流调用抑郁量表(CES-D)和贝克抑郁量表第2版中文版(BDI-Ⅱ-C)在筛查青少年抑郁障碍中的应用效果进行研究。方法分别以3个量表对用整群抽样方法选取的青少年进行抑郁障碍筛查,评价各量表的信度并以临床定式访谈量表(SCID)为金标准评价效度。结果 PHQ-2-C、CES-D和BDI-Ⅱ-C量表的Cronbach’sα系数分别为0.75、0.93和0.84,均>0.6;Pearson’s r系数分别为0.86、0.87和0.79,P<0.05。量表的灵敏度依次为CES-D(95%)>BDI-Ⅱ-C(93%)>PHQ-2-C(89%),特异度依次为BDI-Ⅱ-C(97%)>CES-D(93%)>PHQ-2-C(88%)。结论 3个量表的均具有高度的可靠性和稳定性,PHQ-2-C的灵敏度和特异度稍低,适合作为初期抑郁障碍筛查;CES-D和BDI-Ⅱ-C的灵敏度和特异度相似,适合作为初筛后的进一步筛查。Objective To study the application effects of Chinese Version of the Patient Health Questionnaire 2 （ PHQ-2-C ）, 20-item Centre for Epidemiologic studies of Depression Symptom Scale （ CES-D ） and second edition of Chinese Version of Beck Depression Inventory （ BDI-Ⅱ-C ） in the screening of adolescent with depressive disorders. Methods Adolescents were selected using 3 scales with cluster sampling method for the screening of depressive disor- der. The reliability of each scale was evaluated and SCID was used as the gold standard of validity. Results Cronbach＇s coefficients of PHQ-2-C, CES-D and BDI-Ⅱ -C scale were 0.75, 0.93 and 0.84 respectively, all 〉 0.6; Pear- son＇s r coefficients were 0.86, 0.87 and 0.79 respectively, with P 〈 0.05. The sensitivity of all the scales were CES-D （95%） 〉BDI- Ⅱ-C （93%） 〉PHQ-2-C （89%）, and the specificity wereBDI- Ⅱ-C （97%） 〉CES-D （93 % ） 〉 PHQ-2-C （88 % ） . Conclusion All the 3 scales showed high reliability and stability. Since the sensitiv- ity and specificity of PHQ-2-C are slightly lower, it is considered to be suitable for the early screening of depression. The sensitivity and specificity of CES-D and BDI-Ⅱ-C are similar and considered suitable for further screening after the first screening.
