磁珠分离结合质谱分析黏多糖病Ⅰ型患者尿液多肽谱

摘　　要：目的研究黏多糖病Ⅰ型(MPSⅠ)患者尿液小分子多肽谱,寻找对MPSⅠ型诊断的新方法。方法利用弱阳离子磁珠纯化(magnetic beads based weak cation exchange chromatography,MB-WCX)系统结合基质辅助激光解析电离-飞行时间质谱(matrix-assisted laser desorption-ionization time-of-flight mass spectrometry,MALDI-TOF-MS)的方法,检测8例于解放军总医院儿童医学中心确诊的MPSⅠ型患者(疾病组)和10例本院查体人员(健康对照组)的尿液标本,获得尿液小分子多肽谱,应用Clin Prot TM软件进行数据分析,找出差异多肽峰,并绘制受试者工作特征曲线(ROC曲线),对比曲线下面积(AUC)、敏感度和特异性。结果质荷比1000-10000之间,两组尿液标本共筛选出7个具有统计学差异(P<0.05)多肽差异峰。其中在疾病组中表达上调的有4个峰,表达下调的3个峰。AUC面积大于0.9的多肽质荷比为:1069.6(0.982)和3217.3(0.946),两者的特异性均为87.4%,敏感性均达到100%。结论 MB-WCX联合MALDITOF-MS检测技术可获得MPSⅠ型患者和健康人的尿液差异多肽谱,为MPSⅠ型辅助诊断提供了一种无创的新方法。Objective To analyse the urinary polypeptide pattern of mucopolysaccharidosis Ⅰ（MPS Ⅰ） Methods 8 patients with MPS Ⅰ（MPS Ⅰ group） and 10 healthy people（healthy control group）were enrolled and detected urinary polypeptide pattern by magnetic beads based weak cation exchange separation（MB-WCX） technology and matrix-assisted laser desorption-inoization time of flight mass spectrometry（MALDI-TOF-MS） technology.Clin Prot TM software was used to profile and screen the differential polypeptides in urine,and ROC curve were draw,the area under the curve（AUC）,contrast sensitivity and specificity were analyzed.Result From 1000 and 10000 Da of relative molecular mass,7 significant different（P0.05） polypeptide were detected between MPS Ⅰ group and healthy control goup.Two peptides（m/z：3217.3,1069.6） can be potential biomarker to clarify two groups with a specificity of 87.4%and a sensitivity of 100%.Conclusion MALDI-TOF-MS combined with WCX kit technology is a convenient and high throughput method to selected polypeptide biomarker of MPS Ⅰ,providing a noninvasive new way for screening and diagnosis MPS Ⅰ.
