顾武军教授运用吴鞠通“精确识证”观念治疗肺癌经验

摘　　要：吴鞠通"精确识证"的观念就是要准确认识疾病过程中的症状、证候、体征、病因、病机、疾病和检查检验等多种证据,最大程度地追求治疗方法和用药的准确性和精确性,以提高治疗疾病临床疗效。该文总结了顾武军教授运用"精确识证"观念为指导治疗肺癌的经验,首先应该精确识别正气在肺癌患者治疗中的作用,在放化疗过程中以健脾益肺、扶助正气为主;其次要精确识别痰、毒、瘀在肺癌发生发展过程中扮演的角色,在放化疗后则以涤痰化瘀、抗癌解毒为要,祛邪与扶正兼顾,充分发挥中药的非特异性抗肿瘤作用;再次需精确识别胸腔积液和呼吸道出血等多种并发症,善用仲景《伤寒论》方剂,逐水与止血并举,可明显改善肺癌患者生活质量,延长生存时间。The idea of WU Jutong＇s＂precise identification evidence＂is only to accurately recognize the symptoms,syndromes,signs,etiology,pathogenesis,disease and a variety of evidence and to in the greatest degree pursuit accurate and precise of treatment and medication can improve the clinical efficacy.Summarizes the experience of GU Wujun professors using the idea of WU Jutong＇s＂precise identification evidence＂to guide the treatment of lung cancer was summarized. First of all,the vital Qi in the treatment of patients with lung cancer in chemotherapy should be accurately identify,invigorating spleen and tonifying lung should be used to support healthy;Secondly,the role of phlegm,toxin and stasis in lung cancer occur and development process should be accurately identified,and supporting healthy and removing evil,resolving phlegm,removing blood stasis and detoxification should be applied,which can give full play to the nonspecific antitumor effect of traditional Chinese medicine. Thirdly,we need to accurately identify the pleural effusion and bleeding of respiratory tract,and make good use of the prescription of ZHANG Zhongjing＂Treatise on Febrile Diseases＂to drainage water and hemostasis simultaneously,can significantly improve the life quality and prolong the survival time.
