摘 要:目的研究2型糖尿病患者的总能量消耗量(TEE)及其与患者体内氧化应激水平的相关性。方法以广州市某三甲医院内分泌门诊30~70岁2型糖尿病患者为调查对象,对其24 h体力活动情况进行调查,应用要因加算法求出TEE,采用ELISA法检测尿液中8-羟基脱氧鸟苷(8-OHdG)和8-异前列腺素F2α(8-iso-PGF2α)的浓度以评价调查对象的氧化应激水平,最终采用相关分析及多重线性回归分析研究氧化应激指标与TEE的相关性。结果共调查2型糖尿病患者222例,女性占47.7%(106/222),30~49岁者占48.6%(103/222),广东籍占59.0%(131/222)。男、女性患者的休息和极轻活动之和分别占总活动时间的86.6%、83.4%;男性在轻体力活动上分配的时间少于女性(P<0.01),极轻活动中的坐姿活动多于女性(P<0.01)。男、女性患者的TEE分别为160.4、153.3 kJ/(kg·d),重体力活动水平下各年龄组的TEE均高于相应的RNI值。Pearson相关性分析结果显示,尿中8-OHdG及8-iso-PGF2α均与TEE呈正相关(P<0.05或P<0.01);多重线性回归分析结果显示,8-OHdG及8-iso-PGF2α浓度的影响因素均包括TEE(均P<0.01)。结论重体力活动的2型糖尿病患者的TEE高于其RNI值,TEE可能影响患者尿中8-OHdG及8-iso-PGF2α浓度,有可能增加其体内氧化应激水平。Objective To assess the level of total energy expenditure (TEE) and its association with oxidative stress in patients with type 2 diabetes. Methods Patients aged 30 -70 years with type 2 diabetes were selected from a general hospital in Guangzhou as the subjects. Their information about 24 - hour physical activities were collected to calculate the TEE using the factorial calculation. ELISA was used to detect the concentrations of 8-OHdG and 8-iso-PGF2α in urine to evaluate the level of oxidative stress. Pearson correlation coefficient and multiple linear regression were used to analyze the correlation between the oxidative stress and TEE. Results A total of 222 patients were finally involved in this study. Of them, 47.7% ( 106/222 ) were females 48.6% ( 103/222 ) were aged 30 - 49 years, and 59.0% (131/222) were from Guangdong Province. Rest and very-low activity were accounted for 86.6% and 83.4% of the total time in males and females, respectively. Compared with females, males spent less time on light activity ( P 〈0. 01 ), while more time on sitting ( P 〈 0. 01 ). TEEs were 160.4 k J/( kg · d) for males and 153.3 kJ/( kg · d) for females, and higher in each age group than the corresponding reference nutrient intakes (RNIs) at heavy physical activity level. Pearson correlation analysis showed that the concentrations of 8-OHdG and 8-iso-PGF2α were positively correlated with TEE (P 〈 0. 05 and P 〈 0.01, respectively). Multiple linear regression showed that TEE was an influencing factor for the concentration of 8- OHdG and 8-iso-PGF2α ( both P 〈 0.01 ). Conclusion TEEs of patients with type 2 diabete mellitus and heavy physical activity were higher than RNI, and may affect the concentrations of 8-OHdG and 8-iso- PGF2α in urine and increase the level of oxidative stress.
