叶酸、维生素B_(12)干预对阿尔茨海默病精神障碍患者血浆同型半胱氨酸的影响

摘　　要：目的探讨叶酸、维生素B_(12)干预治疗对伴有精神行为症状的阿尔茨海默病患者血浆同型半胱氨酸浓度的影响。方法对所有的阿尔茨海默病(AD)患者进行血浆同型半胱氨酸(Hcy)、叶酸、维生素B_(12)水平测定,其中BPSD组40例,非BPSD组37例,予叶酸5mg/d和维生素B_(12) 500μg/d干预治疗,8周后检测血浆Hcy、叶酸、维生素B_(12)水平。结果接受叶酸、维生素B_(12)共8周的干预治疗后,BPSD组和非BPSD组的血浆Hcy水平(t=-21.61,t=-13.95)下降,叶酸(t=36.40,t=27.74)及维生素B_(12)水平(t=24.93,t=36.73)升高,差异均有统计学意义(P<0.001),且BPSD组的Hcy下降程度明显高于非BPSD组(t=5.63,P<0.001),但是两组MMSE得分的改善情况并无统计学差异(t=0.59,P=0.559)。结论叶酸、维生素B_(12)干预治疗能显著降低AD患者甚至是BPSD患者的血浆Hcy水平,甚至对伴有精神行为症状的AD患者的疗效更好。Objectives To investigate the effects of folic acid and vitamin B_(12) on the level of Homocysteine( Hcy) from patients with Behavioral and Psychological Symptoms of Dementia( BPSD). Methods In the study,the 77 cases of patients with Alzheimer disease( AD) were divided into BPSD group of 40 cases,and non-BPSD group of 37 cases.. The intervention treatment was conducted in both groups for 8 weeks,and the Plasma homocysteine( Hcy),folic acid and Vitamin B_(12) were measured pre-treatment and posttreatment. Results After treatment,the level of Hcydecreased in the two groups( t =-21. 61,t =-13. 95,all P < 0. 001),and the level of folic acid( t = 36. 40,t = 27. 74,all P < 0. 001) and Vitamin B_(12)( t = 24. 93,t = 36. 73,all P < 0. 001) increased. The decrease value of Hcy in BPSD group was significantly higher than the non-BPSD group( t = 5. 63,P < 0. 001),but the increase value of MMSE scorewas not statistically significant( t = 0. 59,P = 0. 559). Conclusion The intervention treatment of folic acid and Vitamin B_(12) can effectively reduce the plasma levelof Hcy in AD patients,and even has better efficacy in AD patients with BPSD.
