《伤寒杂病论》“病脉证并治”诊疗思维模式临床应用

摘　　要：目前中医收集病理资料的方式依然是望闻问切,形式上与古人相同,但在具体操作上却相差较大。现代中医过分依赖问诊,主要依靠病人陈述,望闻问切结束方开始分析病情;而古代大医却较重视脉诊,参考问诊,脉诊的过程即是诊断辨证的过程,相比之下,此法对医生的诊断水平要求较高,但诊断的准确性亦较高,张仲景《伤寒杂病论》的"病脉证并治"的诊疗模式当为此诊法之肇端。"脉诊"乃"病脉证治"诊疗模式的核心内容。脉诊是对患者整个身体状态的把握,其作用主要表现在:推测病因,推测治疗经过,推测兼证,推测预后,指导治疗,其中,最关键是治疗作用。由此可见,较目前中医普遍应用的诊疗模式而言,张仲景《伤寒杂病论》中体现的"病脉证并治"诊疗思维模式临床的应用价值更大。At present,Chinese medicine pathological data is collected is still through inspeetion,listening and smelling examina- tion,interrogation,pulse-taking and palpitation. The way is similar to that of ancient, but differs in the specific operation. Modem TCM physicians rely on too much interrogation,that is patient＇s statements,and begin to analyze disease after usingthe above four diagnostic methods, while the ancient medical masters paid more attention to pulse diagnosis, consulted interrogation, so the process of pulse-taking is a process of dialectical diagnosis. In contrast,the diagnosis＇ request for the doetor＇s is higher, but the di- agnostic accuracy is also higher,and Zhang Zhong-jing begins the ＂ treatment based on disease and pulse and syndrome together＂ thinking mode of diagnosis and treatment. ＂ Pulse＂ is the core. pulse-taking and palpitation can completely grasp the patient＇s whole body state, and its role is mainly manifested in inferring disease cause, treatment process,accompanying syndromes, progno- sis and guiding treatment. Among them,the most critical is the treatment effect. Thus,compared with the eurrent commonly- used TCM treatment mode ,the application value of ＂treatment based on disease and pulse and syndrome together＂ thinking mode of di- agnosis and treatment is much higher.
