阿尔茨海默病患者睡眠模式与认知功能的相关研究

摘　　要：目的通过对AD患者进行PSG和认知功能的评定,研究AD患者的睡眠模式特点以及睡眠模式和认知功能的相关性。方法随机选取我院收治的29例AD患者作为AD组,另选取24例健康老年人作为对照组。于晚上进行PSG检查,并同日采用MMSE和ADAS-Cog量表评定。结果 (1)与对照组相比,AD组的AST、REMST和总NREM减少,总觉醒时间和WASO增多,SE降低,差异均有统计学意义(P<0.05)。(2)与中度AD患者相比,重度AD患者的AST、REMST和总NREM减少,总觉醒时间延长,SE降低,差异均有统计学意义(P<0.05)。(3)AD组的总觉醒时间、REMSL与MMSE评分呈负相关(P<0.05)。结论 AD患者存在明显的睡眠结构障碍,且随着病情的加重而更紊乱,与认知功能有一定关系。Objective To study the characteristics of sleep pattern, cognitive function and the relationship between sleep pattern and cognitive function by applying PSG and cognitive function evaluation on AD patients. Methods 29 cases of AD patients （AD group） admitted to our hospital were randomly selected, and another 24 cases of healthy elderly people were selected as the control group. Both groups were examined at night for PSG and evaluated by the MMSE and ADAS-Cog. Results （1）Compared with the control group, AD group had decreased AST, REMST, total NREM and SE, and increased total awakening time and WASO, with statistical differences （all P 〈0.05）. （2）Compared with the moderate AD patients, the severe AD patients had decreased AST, REMST, total NREM and SE, and increased total awakening time, with statistical differences （all P 〈0.05）. （3）The total awakening time and REMSL of AD group were negatively correlated with MMSE score （P 〈0.05）. Conclusions AD patients have obvious sleep structure disorder. Along with the progression of clilical festations, the severity of sleep structure disorder in AD patients has a tendency to become more severe. The sleep structure disorder is exactly correlated with the cognitive function in AD patients.
