摘 要:目的探讨早期肠内营养对胃癌根治术患者术后免疫功能及抗氧化功能的影响。方法 80例胃癌根治术患者分为观察组和对照组各40例,术后均给予常规静脉滴注营养,在此基础上,观察组给予肠内营养,疗程为1周;比较两组患者术前1天和术后第8天营养状况(白蛋白水平、前白蛋白水平及体重)、免疫指标(CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+比值、NK细胞的比例和IFN-γ/IL-4)、抗氧化功能(T-SOD、MDA、GSH-Px)。结果术前1天,2组患者的营养状况、免疫指标及抗氧化功能差异无统计学意义(P>0.05)。术后第8天,观察组患者CD_8^+T细胞降低,CD_4^+/CD_8^+、NK细胞和GSH-Px水平升高,差异均有统计学意义(P<0.05);其他指标与术前比较差异无统计学意义(P>0.05)。对照组患者术后第8天体重、CD_3^+、CD_4^+、CD_4^+/CD_8^+、IFN-γ/IL-4降低,MDA水平升高,与术前比较差异均有统计学意义(P<0.05)。结论胃癌根治术后给予早期肠内营养有利于维持患者术后的免疫功能和抗氧化-氧化系统的平衡,促进术后恢复。ObjectiveTo understand the effect of early enteral nutrition on the postoperative immune function and antioxidant function of patients of gastric cancer undergoing radical gastrectomy. Method Firstly, 80 patients of gastric cancer undergoing radical gastrectomy were divided into an experimental group (n = 40) and a control group (n = 40). Both groups were given routine postoperative intravenous nutrition. In additionto intravenous nutrition, the experimental group were given enteral nutrition for one week.The nutritional status (prealbumin level, body weight and albumin level) , immune indexes ( CD3+ , CD4~ , CDs~ , CD4~/CDs~ ratio, NK cell ratio and IFN- gamma / IL-4) , antioxidant function (T-SOD, MDA, GSH-Px) as observed one daybefore operation and eighth days after operation inthe two groups were compared. Findings Before operation, no significant difference was found between the two groups in nutritional status, immune index and antioxidant function (P〉0.05). Eighth days after operation, the experimental group were found to have adrop in CD8+T cells and an increase in the levels of CD4+/CD8+ , NK and GSH-Px ceils. The differences werestatistically significant( P〈0. 05). No significant differences were found in other indexes before and after treatment (P〉0.05). In the control group, the weight, CD3+, CD4+, CD4+ /CD8+, IFN-, gamma / IL-4decreased and the level of MDAincreasedeighth days afteroperation. The difference was statistically significant (P 〈 0.05 ). Conclusion Early enteral nutrition after radical gastrectomy helps maintain postoperative immune function and balance antioxidant oxidation system and promote postoperative recovery.
