古今痔病病证结合模式演变规律研究

摘　　要：病证结合模式是疾病中医诊疗观由"辨病"向"辨证与辨病结合"转变的产物,也是客观评价中医疾病疗效的科学方法之一。影响痔病病证结合模式的决定因素有痔病中医诊疗观的演变、古今痔概念内涵的演变规律、痔病相关证的时相性和空间性特征,以古今痔概念的演变规律及证的时相性特征为经,以证的空间性特征为纬,可将古今痔病病证结合模式分为3种模式,即秦汉古痔局部病证结合模式、明清古痔整体病证结合模式、近现代痔病病证结合模式,3种模式均有缺陷,有必要重构融合痔病病理学本质及痔病内外证各自时相性和空间性特征的现代痔病证结合模式,为客观评价痔病中医临床疗效奠定方法学基础。Combination of disease and syndrome mode （CDSM） is a outcome of which TCM doctor＇ concept on disease shift opinion from differentiation disease to combination of differentiation disease and syndrome, while CDSM is also one of science methods which can objectively evaluate the therapeutic effect of TCM. Deletion of research on CDSM affect the work of objective evaluation of therapeutic effect of TCM of haemorrhoids. There are three factors that affect evolvement rule of haemorroidal CDSM,CDSM evolvement of TCM doctor＇ concept on haemorrhoids;Evolvement Rule of Ancient and Mordern concept of haemorrhoids;Time phase and spatislity of syndrome related haemorrhoids. According to the later two,CDSM of haemorrhoids in ancient and modern times can be divided into three modes,local CDSM of QinHan ancient haemorrhoids; whole CDSM of ancient haemorrhoids in Ming and Qing dyrasty; CDSM of modern and contemporary times. But there are some flaws in all of the three modes. To evaluate the therapeutic effect of TCM objectively,it is necessary that modern CDSM of haemorrhoids which merge pathology nature of haemorrhoids and respective time phase and spatislity of haemorrhoidal inter and outer syndrome together is reconstituted.
