摘　　要：目的探讨检测错配修复蛋白(MMR)对初步筛查有遗传倾向内膜癌Lynch综合征患者及对其家系成员患者肿瘤的预防意义。方法选取2011年3月至2016年9月间苏州大学附属第三医院收治的100例子宫内膜癌患者,采用免疫组织化学方法检测子宫内膜癌患者子宫内膜癌组织中MMR(包括MLH1,MLH2,PMS2和MLH6)表达缺失情况,并分析其与患者发病年龄、病理类型、分化级别、肌层浸润程度、淋巴结是否转移及临床分期之间的关系。结果 MMR蛋白表达缺失率为22.0%,年龄≤50岁组、高级别病理分化组、>1/2肌层组和有淋巴转移组的MMR表达缺失率高于年龄>50岁组、低级别病理分化组、≤1/2肌层组和无淋巴转移组,差异均有统计学意义(均P<0.05)。不同临床分期各组的MMR表达缺失率比较,差异无统计意义(P>0.05)。结论年龄≤50岁、高级别分化、>1/2肌层浸润及存在淋巴转移的子宫内膜腺癌患者,MMR蛋白的表达缺失率高,其检测可用于临床初步筛查子宫内膜癌Lynch综合征患者及对其家系成员肿瘤的早预防。Objective To investigate the expression of mismatch repair protein（ MMR） in endometrial carcinoma the significance of MMR in the initial diagnosis and prevention of genetic predisposition lynch syndrome. Methods One hundred patients with endometrial cancer were selected at The Third Affiliated Hospital of Soochow University from March 2011 to September 2016. Immunohistochemistry was used to detect the expression deficiency of MMR（ including MLH1,MLH2,PMS2 and MLH6）. The relationship between age of onset,pathological type,differentiation grade,myometrial invasion,lymph node metastasis and clinical stage was analyzed. Results The expression deficiency rate of MMR was 22. 0%. The expression deficiency rate of MMR was higher in patients≤50 years,high pathological differentiation, 〉1/2 muscle and with lymph node metastasis（ all P〈0. 05）. There was no significant difference in the expression of MMR between different clinical stages（ P〉0. 05）. Conclusion For endometrial carcinoma patients at age of ≤50 years old,with high differentiation and 〉1/2 myometrial invasion and lymph node metastasis,the expression deficiency rate of MMR is high. The detection can be used in clinical screening and early prevention of genetic predisposition lynch syndrome.
