摘 要:目的观察现场"督导"教学方法在心血管科住院医师规范化培训带教中的实施效果。方法选取2015年3—2017年10月在蚌埠医学院第一附属医院心血管科进行规培的学员120名作为研究对象,随机分为对照组和实验组,每组60名,实施传统教学方法组作为对照组,采用现场"督导"教学方法组作为实验组,教学结束后对两组学员采用统一理论考核和临床实践技能考核,并发放统一的调查问卷,了解两组学员对教学的认可程度,比较两组教学效果。结果实验组学员的病例分析[(24.36±1.96)分vs.(21.84±1.25)分,P<0.05]、技能操作考核成绩[(24.04±2.01)分vs.(21.32±2.04)分,P<0.05]和总成绩[(84.04±3.13)分vs.(78.08±2.16)分,P<0.05]均高于对照组,理论考核两组比较差异无统计学意义[(35.64±1.47)分vs.(34.92±1.96)分,P>0.05]。实验组学员的学习自主能力[(4.44±0.58)分vs.(3.12±0.78)分,P<0.05]、解决临床问题能力[(4.32±0.75)分vs.(3.24±0.52)分,P<0.05]、学习兴趣[(4.28±0.74)分vs.(3.20±0.65)分,P<0.05]、医患沟通、表达能力[(4.20±0.71)分vs.(3.56±0.51)分,P<0.05]等方面均高于对照组,团队协作能力两组比较差异无统计学意义[(3.28±0.74)分vs.(3.24±0.66)分,P>0.05]。结论采用现场"督导"教学模式可提高心血管科住院医师规范化培训学员的临床实践技能,提升学员对教学的认可程度,增强学员的综合能力,值得在住院医师规范化培训中进一步推广和应用。Objective To investigate the application effect of the"supervision"teaching model on the spot in resident standardization training in the department of cardiology. Methods A total of 120 students for resident standardization training from March2015 to October 2017 were divided into experimental group and control group,with 60 students in each group. The "supervision " teaching model on the spot was applied to the experimental group,while the control group received the traditional approach. We carried out the theoretical and clinical practice skills tests at the end of the teaching and questionnaire survey was used to understand the residents for teaching evaluation. Results The results of case analysis( 24.36±1.96 vs. 21.84± 1.25,P〈0.05),practical skill assessment( 24.04±2.01 vs. 21.32±2.04,P〈0.05) and total score( 84.04± 3.13 vs. 78.08±2.16,P〈0. 05) in experimental group were higher than those of control group. There was no significant difference between the two groups in theoretical examination( 35. 64 ± 1. 47 vs. 34. 92 ± 1. 96,P〈0. 05). The self-learning ability( 4.44±0.58 vs. 3. 12 ± 0. 78,P〈0. 05),problem-solving ability( 4. 32 ± 0. 75 vs. 3. 24 ± 0. 52,P〈0. 05),learning interest( 4.28±0.74 vs. 3.20±0.65,P〈0.05) and communication ability between doctors and patients( 4.20±0.71 vs. 3.56 ± 0. 51,P〈0. 05) in the experimental group were all higher than those in the control group.There was no significant difference between the two groups in teamwork ability( 3.28± 0.74 vs. 3.24± 0.66,P〈0.05). Conclusions The "supervision"teaching model on the spot can increase the clinical practice skills of residents,improve students' satisfaction of teaching and enhance the comprehensive ability of students,which is worthy of further popularization and application.
