青少年心理行为问题与青春期发动时相提前以及肥胖的相关性

摘　　要：目的:探索肥胖和青少年青春发动时相提前与心理行为问题的相关性。方法:选取郑州地区9所城乡中小学校10～18岁青少年共3143人,采用青春发育量表(PDS)评价青春发动相关事件,长处和困难问卷(SDQ)评定心理行为问题。以青春发育得分的75%作为青少年青春期发动时相提前的划分标准,以BMI的95%作为肥胖的检出标准。结果:男女生青春发动时相提前组肥胖率均大于发动时相正常组(16.4%vs.8.5%;12.6%vs.5.8%)(P<0.05)。多重线性回归分析显示,多动因子和亲社会因子与青春发动时相相关(R_(adj)~2=0.133,P<0.001;R_(adj)~2=0.123,P<0.001),情绪因子与青春发动时相和年龄相关(R_(adj)~2=0.231,P=0.047,P<0.001),品行因子和同伴交往因子育青春发动时相、性别和年龄相关(R_(adj)~2=0.252,P=0.001,P<0.001,P<0.001;R_(adj)~2=0.230,P=0.007,P<0.001,P=0.001)。结论:青少年心理行为问题中的五种因子均与青春发动时相提前具有相关性,且情绪因子、品行因子和同伴交往因子同时也与性别和年龄相关联。Objective： To examine the correlation of behavioral and emotional problems and early puberty,obesity in adolescents. Methods： A total of 3143 children and adolescents aged 10-18 years old from 9 urban and rural primary and secondary schools in Zhengzhou City,the Puberty Development Scale（PDS） was used to evaluate youth-related events,Strengths and Difficulty Questionnaires（SDQ） were used to assess psychological and behavioral problems. The 75 percent of pubertal development score was used as the criterion for puberty and the 95 percent of BMI was used as the criterion for the detection of obesity. Results： Obesity rates of boys and girls were greater in the early stage of puberty initiation group than in the normal group（ 16. 4% vs. 8. 5%; 12. 6%vs. 5. 8%）（P〈0. 05）. The multiple linear regression analysis showed that hyperactivity/inattention and pro-social behaviors were mainly influenced by pubertal timing（R（adj）2= 0. 133,P〈0. 001; R（adj）2= 0. 123,P〈0. 001）,emotional problems were influenced by both pubertal timing（R（adj）2= 0. 231,P = 0. 047,P〈0. 001）. Conduct problems and peer relationship problems were both affected by age and pubertal timing,age and gender（R（adj）2=0. 252,P = 0. 001,P〈0. 001,P〈0. 001; R（adj）2= 0. 230,P = 0. 007,P〈0. 001,P = 0. 001）. Conclusion：The five factors in psychological and behavioral problems of children and adolescents are all related to early puberty,and emotional problems,conduct problems and peer relationship problems are also related to gender and age.
