摘 要:目的:探讨视觉反馈环境下减重运动控制训练对脑卒中偏瘫患者下肢运动功能障碍和移动能力改善的有效性。方法:本研究纳入34例脑卒中患者,通过随机化数字表将其随机分为两组,对照组接受20次电动起立床(或站立架)训练,治疗组接受20次视觉反馈环境下的减重运动控制训练。在治疗前、治疗2周时和治疗4周后分别应用下肢Brunnstrom量表、下肢简化Fugl-Meyer量表、Berg平衡量表、River-mead活动性指数、功能步行能力量表和Barthel指数对受试者的下肢运动功能、平衡功能、移动能力和日常生活活动能力进行评估。结果:治疗2周后和4周后,两组各项功能评分均较其治疗前显著改善(P<0.05);治疗组的Brunnstrom分期、Berg评分、Rivermead评分、功能步行能力评分和Barthel指数均较对照组显著改善(P<0.05,P<0.001)。结论:视觉反馈环境下减重运动控制训练能有效改善脑卒中偏瘫患者下肢运动功能、平衡功能、移动能力和日常生活活动能力。Objective To investigate the effect of motor control training in weight relief state withvisual feedback on improving the motor impairment of lower extremity and mobility in hemipareticstroke patients. Methods Thirty-four stroke survivors were enrolled and randomized into a controlgroup and a treatment group. Participants in the control group received conventional standing trainingfor 20 times while those in the treatment group received motor control training in weight relief statewith visual feedback for 20 times. Assessments were conducted on the motor function of lower extremi-ty,balance,mobility and activity of daily life before,2 weeks and 4 weeks after the treatment usingBrunnstrom of lower extremity(BLE),simplified Fugl-Meyer of lower extremity,Berg balance scale(BBS),Rivermead mobility index(RMI),functional ambulation category(FAC) and Barthel index(BI).Results After 2-week and 4-week treatment,significant improvement was observed in all the measure-ments of both groups(P〈0.05). However,the improvement of the average score of BLE,BBS,RMI,FAC and BI was more significant in the treatment group,compared with that of the control group. Conclusion Motor control training in weight relief state with visual feedback significantly improves the motorfunction of lower extremity,as well as the balance,mobility and activity of daily life of hemipareticstroke survivors.
