摘  要:目的:促进基本药物在临床的优先。方法:基于实证调查和文献研究,从我国基本药物临床使用环节存在的问题入手,以利益相关者理论为指引,以部分省份的先进经验为支撑,提出构建我国医疗机构基本药物使用激励机制的可行性思路。结果:单一的激励约束手段在促进基本药物优先使用方面虽然发挥了一定的积极效果,但未充分调动政府、医疗机构、患者各方合力,激励机制的可持续性受限,也容易产生相互制肘。结论:建立可持续高效的激励机制应当首先消除各利益相关者在促进基本药物优先使用环节中存在障碍性因素,采取激励与约束协同的手段,建立多方参与的基本药物优先使用激励体系。Objective To promote the priority use of essential medicines in clinic. Method Based on the empirical investigation and literature review, started from the problems in the ehnical use of essential drugs, guided with the stakeholder theory and supported by the advanced experience in some provinces, the feasible scheme was put forward. Results A single incentive and restraint approach has played a positive role in promoting the priority use of essential medicines, but it has failed in fully mobilizing the government, medical institutions and patients. The sustainability of incentive mechanism was limited. And it is possible to result in mutual restraints. Conclusion To establish the sustainable and efficient incentive mechanism for the priority use of essential medicines, it should eliminate the obstacles of various stakeholders, and adopt synergic means of incentive and restraint.
