摘 要:目的探究四川片区先天性甲状腺功能减低症(CH)发病率与季节的关系。方法 2015年1月至2016年12月,四川省新生儿疾病筛查中心采用时间分辨荧光法检测536040例新生儿足跟滤纸干血片促甲状腺素(TSH)浓度,对于通知确诊的患儿检测静脉血血清TSH和游离甲状腺素(FT4)水平,按不同月份和不同季节分组,比较新生儿初筛阳性率、召回率、失访率及发病率,并对结果进行分析。结果共确诊患儿256例,其中CH 196例、高TSH血症60例。不同月份初筛阳性率和召回率差异有统计学意义(P<0. 05),CH和高TSH血症总发病率、CH发病率差异无统计学意义(P>0. 05);不同季节初筛阳性率和召回率差异有统计学意义(P<0. 05),失访率、CH和高TSH血症总发病率、CH发病率差异无统计学意义(P>0. 05);春冬季与夏秋季相比,初筛阳性率和召回率差异有统计学意义(P<0. 05),失访率、CH和高TSH血症总发病率、CH发病率差异无统计学意义(P>0. 05)。结论四川片区先天性甲状腺功能减低症发病率与季节不相关,可能与切值使用不合适和召回率不同等因素有关。Objective To investigatethe relationship between morbidity of congenital hypothyroidism( CH) and season in Sichuan. Methods During January 2015 to December 2016,Thyroid Stimulating Hormone( TSH) of 536040 newborns in dried blood spots were determined using time-resolved fluorescence method by neonatal disease screening center of Sichuan province. As for confirmed child patients,serum TSH and free thyroxine( FT4) in venous blood were determined. They were also divided into different groups according to different months and seasons,and child patients in different groups were compared in terms of positive rate of initial screening,recall rate,the rate of loss to follow-up and morbidity,the results being analyzed. Results 256 child patients were confirmed,among which there were 196 cases of CH and 60 cases of hyperthyrotrophinemia. There were statistically significant differences in positive rate of initial screening and recall rate among different months( P〈0. 05),while there was no statistically significant differences in total morbidity of CH and hyperthyrotrophinemia as well as morbidity of CH among different months( P〉0. 05). There were statistically significant differences in positive rate of initial screening and recall rate among different seasons( P〈0. 05),while there was no statistically significant differences in the rate of loss to follow-up,total morbidity of CH and hyperthyrotrophinemia as well as morbidity of CH among different seasons( P〉0. 05). Comparing conditions in spring and winter with conditions in summer and autumn,there were statistically significant differences in positive rate of initial screening and recall rate( P〈0. 05),but no statistically significant differences in the rate of loss to follow-up,total morbidity of CH and hyperthyrotrophinemia as well as morbidity of CH among different seasons( P〉 0. 05).
