公立医院外部监管的逻辑起点探析与理论框架构建

摘　　要：依据黑格尔和马克思等关于逻辑起点规定性的要求,运用逻辑与历史相统一的方法,讨论了公立医院行为作为公立医院外部监管理论体系逻辑起点的合理性,认为公立医院行为是公立医院外部监管体系中最直接、最简单的起始范畴,是构成公立医院外部监管体系的细胞和元素。基于公立医院行为的历史惯性、医疗卫生服务的特殊性以及医疗服务市场的结构特征,分析了公立医院外部监管实践过程中可能的理论依据,包括博弈论理论、委托代理理论、利益相关者理论、多中心治理理论等,并以此为基础构建了以公立医院行为为逻辑起点的公立医院外部监管理论体系框架,为研究公立医院外部监管、建立现代医院管理制度提供了新的依据。According to Hegel and Karl Marx＇s basic requirement about the logic starling point, we use dialectical logic methods, such as the unillcation and history,from the abstract to concrete,to analyze the rationality respecting public hospitals behavior as logical starting point of external monitoring mechanism fot public hospitals. We think that public hospitals behavior is the most direct, the simplest and abstract starting category in the external monitoring mechanism fot public hospitals. Also, it is the cells ,elements and the end of the systems, and consistent with the historical point. From the study view and purpose, We analyze the theoretical basis construct the hierarchy for the procession of external moni- toring mechanism fot public hospitals. These may supply a new paradigm fot studying the external monitoring mechanism lot public hospitals.
