摘  要:目的研究分析全瓷嵌体行激光预处理后的生物学黏结性能。方法收集由于阻生或牙周病拔除的无缺损、无龋人体新鲜第三磨牙60颗。以数字法随机分成空白参照组(15颗)、810 nm激光组(15颗)、980 nm激光组(15颗)、水激光组(15颗)。4组各选取1颗,就全瓷嵌体与牙表微结构,经几种激光(810 nm、980 nm、水激光)不同预处理后所产生的影响展开研究;4组各选取6颗,研究不同激光预处理后全瓷嵌体和牙本质黏结性能的影响;4组各选取8颗,研究不同激光预处理后牙面与全瓷嵌体间微渗漏情况。结果空白参照组,瓷表面形态较清晰;810 nm激光组,瓷表面结构较松散;980 nm激光组及水激光组,瓷表面形态光滑细腻。空白参照组,牙表牙本质未出现开放小管口,牙本质管间较管周颜色浅;810 nm激光组,牙本质小管清晰可见且管口大;980 nm激光组,牙本质均出现半开放小管口;水激光组,位于牙本质外表呈片状及鱼鳞状,牙本质均出现开放小管口。对牙本质外表进行处理(不同程度蚀刻)后,黏结抗剪切强度均高于空白参照组[(1.03±0.33) MPa],水激光组[(6.66±2.21) MPa]上升最为明显,较其他3组相比,差异均有统计学意义(P <0.05);剩余2组黏结抗剪切强度[(2.34±0.83) MPa]和[(2.03±0.67) MPa]相似,两组间差异无统计学意义(P> 0.05)。不同牙本质表面预处理后4组断裂试件数,差异无统计学意义。4组间微渗漏深度及间隙宽度,差异均无统计学意义(P> 0.05)。结论采用水激光蚀刻处理牙本质,能明显提高牙齿与全瓷嵌体的黏结强度,但对牙齿与全瓷嵌体的密合性是否会有影响还需研究。Objective To study the biological bonding properties of all-ceramic inlay after laser pretreatment. Methods A total of 60 fresh third molars without caries, pulled due to dental impaction or periphery were selected and randomly divided into 4 groups(blank reference group, 810 nm laser group, 980 nm laser group and water laser group, n = 15). In each group,one tooth was selected to study effects of all-ceramic inlays and dental micro-structures after different pretreatments of several lasers(810 nm, 980 nm and water laser). Six teeth in each group were selected to study the effects of all-ceramic inlays and dentin bonding performances after different laser pretreatment. Eight teeth in each group were selected to study microleakage between dentin surface and all-ceramic inlays after different laser pretreatment. Results The surface morphology of porcelain was clear in blank reference group, loose in 810 nm laser group, smooth and fine in 980 nm laser group and water laser group.In blank reference group, there was no opening orifice in dentin surface, the color of dentinal canals was lighter than that of peritubular. In 810 nm laser group, dentine orifices were clearly visible and large. In 980 nm laser group, semi-opening orifices were observed in all dentine. In water laser group, dentine surface was flaky and fish-scale, with opening orifice in dentin. The bonding shear strengths of the treated dentin surface(etched in different degrees) were higher than that of blank reference group[(1.03 ± 0.33) MPa], with the most obvious increase in water laser group[(6.66 ± 2.21) MPa], compared with 3 groups, the difference was statistically significant(P 〈0.05). The bonding shear strength of 810 nm laser group and 980 nm laser group[(2.34 ± 0.83) MPa and(2.03 ± 0.67) MPa] was similar, while there has no significant difference between 2 groups(P〉 0.05). There was no significant difference in the number of breakage in 4 groups after pretreatment with different dentin surfaces. There
