女性前牙反患者颅底及鼻上颌复合体发育趋势的二维有限元法研究

摘  要:目的 用有限元法分析女性前牙反患者从乳牙期到替牙期颅底及鼻上颌复合体发育趋势。方法 用专门建立的颅颌面形态有限元分析系统 ,分析 82例女性前牙反患者从乳牙期到替牙期颅底及鼻上颌复合体发育趋势。结果 从乳牙期到替牙期 ,后颅底与前颅底的关系比较稳定 ,随年龄变化很小。上面部的后上份发育较好 ,而前份发育相对滞后。鼻骨的长度和高度增加趋势明显。腭骨面积有减小趋势 ,可能导致面中部发育不足。结论 有限元法 (FEM)目前虽然不能取代X线头影测量法(RCM) ,但可以作为RCM重要的、有益的补充 ,使对颅颌面形态的分析更科学、更全面、更细致。用FEM进行人颅颌面生长发育研究 ,其结果是恒定的 ,与坐标系的选择和参照平面无关。Objective To analyze the growth and development tendency of cranial base and nasal-maxillary complex of female with anterior cross-bite from deciduous dentition period to mixed dentition period Methods Two-dimensional finite element analysis was performed in 82 above mentioned cases with a specially developed computer software. Results From deciduous dentifion period to mixed dentition period, the posterior cranial base and the anterior cranial base had a relatively steady relation, little changes occurring with aging. The upper-posterior part of upper face grew better, while the upper-anterior part grew less relatively. The length and height of nose bone showed a pronounced increasing tendency. The tendency of decrease of the area of the palatal bone may lead to the underdevelopment of the mid-part of the face. Conclusion Although finite element method could not replace RCM, the former could be an important and good complement of the later, which made the analysis of the cranialfacial morphology more scientific, more complete and more meticulous. The results achieved by FEM were stable, and irrelevant to coordinate system and reference plane.
