摘  要:目的:观察特异性COX-2抑制剂Celecoxib对重症急性胰腺炎肠道屏障的保护作用。方法:雄性Wistar大鼠72只,随机分为假手术组(SO组)、重症急性胰腺炎组(SAP组)及治疗组。采用胰胆管逆行注入5%牛磺胆酸纳复制Wistar大鼠坏死性胰腺炎模型。造模前6、2 h分别给大鼠灌胃注入选择性COX-2抑制剂Celecoxib后,分别在3、6、12h采血,对各组动物行血清淀粉酶、血清D-乳酸、内毒素、COX-2表达以及腹腔脏器移位率进行检测。结果:SAP组血清淀粉酶、血清D-乳酸、内毒素、COX-2表达以及腹腔脏器移位率较假手术组有升高,而治疗组各项检测指标较SAP组有降低。结论:选择性COX-2抑制剂Celecoxib可以降低坏死性胰腺炎大鼠肠道COX-2的表达,从而起到保护肠黏膜屏障的作用。Objective:To investigate the protective roles of specific cyclooxygenase-2(cox-2) inhibitor(celecoxib) in the intestinal mucosal barrier injury of severe acute panereatitis.Methods:Seventy-two Wistar rats were randomly divided into sham-operation(SO) group,SAP group,treatment group.SAP was induced in rats by injection of 5% sodium taurocholate into biliopancreatic duct.Plasma endotoxin and D-lactate were determined at 3,6 and 12 hour after operation.Cyclooxygenase-2 in the intestinal mucosa was detected with immunohistochemistry at 3,6,12 hours after operations.Results:The plasma endotoxin and D-lactate in SAP group increased obviously compared with the of SO group(P<0.05).The plasma concentrations of endotoxin and D-lactate increased stabilizedly after operation in SAP group.The treatment group markedly decreased the plasma endotoxin and D-lactate level compared with that of SAP group(P<0.05),and also ameliorated the histologic injury of pancreas and intestinal mucosa of SAP.Expression of cyclooxygenase-2 was almost absent from intestinal mucosa in the SO group but remarkably increased in SAP group.Conclusion:Specific cyclooxygenase-2(cox-2) inhibitor(celecoxib) might play a protective role in the intestinal mucosal barrier injury of severe acute panereatitis.
