摘  要:目的探讨青光眼患者角膜内皮细胞形态计量学变化及其临床意义。方法对32例原发性闭角型青光眼(PACG)和26例原发性开角型青光眼(POAG)患者进行了角膜内皮显微镜观察和计算机图像分析。结果与正常人相比PACG患者角膜内皮细胞(CEC)密度、形状因子明显减小,面积、周长、最大长径、变异系数明显增大(P<0.01),POAG患者CEC形状因子减小(P<0.05)而其它各检测指标无明显改变。结论PACG对CEC结构及其稳定性有明显影响,眼压应尽早控制,对PACG和POAG患者进行各类内眼及角膜手术时,围手术期的CEC形态计量学检测十分重要。Objective: to study the morphometric changes of corneal endothelialcells (CEC)in glaucoma and to evaluate the clinical significance of these changes. Methods: The CEC of 32 patients with primary angle closure glaucoma(PACG) and 26 patients with primary open-angle glaucoma (POAG) were examined by specular microscopy and computer-aid image analysis system. Results; As compared to the normal controls,the CEC of PACG showed an obviously decreased cell density(D) and form factor(FF) (p<0.01)and an increased area(A), perimeter(Pe), long axis (Lmax) and coefficience of variation (CV) (p<0.01). The CEC of POAG showed a decreased FF (p<0.05) but there were significant differences discovered in D, A, Pe, Lmax and CV between POAG and the normal controls.Conclusions:PACG may greatly affect the corneal endothelial structure and its stabilily. Therefore,intraocular pressure should be controled as early as possible.And,the quantitative morphological analysis of the CEC in patients with PACG or POAG during the coneal or intraocular perioperative period is very important.
